卵母细胞玻璃化冷冻的临床结局分析

摘　　要：目的探讨卵母细胞玻璃化冷冻后解冻行体外受精-胚胎移植的临床结局。方法回顾性分析2010年1月至2014年10月北京大学第三医院生殖医学中心68例不孕症患者卵母细胞玻璃化冷冻后解冻行体外受精-胚胎移植(in vitro fertilization-embryo transfer,IVF-ET)的复苏率、2PN受精率、胚胎着床率、流产率和分娩率等临床结局。结果解冻玻璃化冻存的卵母细胞756枚,复苏率80.69%(610/756),其中MII卵母细胞519枚,2PN受精率53.56%(278/519)。68例患者中,12例无胚胎形成,占17.65%。56例患者共移植66个周期,其中新鲜胚胎移植51个周期,解冻胚胎移植15个周期。新鲜胚胎移植51个周期的临床妊娠率为54.90%(28/51),活胎分娩率为45.10%(23/51)。解冻胚胎移植15个周期的临床妊娠率为60.00%(9/15),活胎分娩率为46.67%(7/15)。累积妊娠率为55.88%(38/68)。每解冻一枚卵母细胞获得活胎分娩率为5.03%(38/756)。结论卵母细胞玻璃化冷冻技术在取卵日不能获得精子的患者中应用,可以获得良好的妊娠结局。Objective To investigate the clinical outcome of IVF- ET cycle using vitrified-warmed oocyte in infertility couples. Methods Retrospective analysis the clinical outcome of IVF-ET cycles using vitrified-warmed oocytes in the Third Hospital of Peking University from 2010 to 2014. The primary measurement is clinical pregnancy rate and the delivery rate, the second measurement includingsurvival rate, normal fertilization rate （2PN）, and implantation rate. Results 756 vitrified oocytes （68 couples） have been thawed, the survival rate is 80.69% （610/756）. 519 MII oocytes were inseminated and 2PN fertilizing rate is 53.56% （278/519）. 12 of 68 patients had no embryos for transfer （17.65%）. The remaining 56 couples undergoing the embryo transfer, included 51 fresh embryo transferand 15 frozen- thawed embryotransfer. The clinical pregnancy rate in fresh embryo transfer cycle is 54.90% （28/51）, the live birth rate is 45.10% （23/51）. In another 15 frozen-thawed embryo transfer cycles, the clinical pregnancy rate and the live birth rate is 60.00% （9/15） and 46.67% （7/15）, respectively. The cumulative pregnancy rate of oocyte vitrification is 55.88% （38/68）. Each vitrified oocytes obtained live birth rate is 5.03% （38/756）. Conclusions Oocytes vitrification technology may acquire the acceptable clinical outcome in the patients who cannot get sperm on the day of oocyte retrieval.The safety of oocyte vitrification need further investigation.
