摘 要:目的:探索影响针刺效应的主次因素和针刺参数的最佳组合,以期为提高针刺疗效、指导针灸临床提供理论依据。方法:采用L9(34)正交表对影响针刺效应的4因素(穴位的选择、捻针角度、捻针频率、捻针时间)3水平安排试验,运用计算机控制针刺手法仪治疗肾阳虚大鼠,通过酶联免疫检测血清T、E2和实时荧光定量PCR检测垂体中LH、FSH mRNA表达的水平来比较上述4因素的主次。结果:上述4因素对针刺效应的影响大小依次是穴位选择、捻针角度、捻针频率、捻针时间,其中腧穴的选择是影响针刺效应的重要因素(P<0.05);捻针角度、捻针频率和捻针时间是影响针刺效应的次要因素(P>0.05)。结论:针刺治疗肾阳虚大鼠的最佳针刺参数为针刺肾俞穴,捻针角度为90°,频率为60次/min,捻针60s;不同因素对针刺治疗肾阳虚大鼠的疗效不同,其中穴位的选择对疗效的影响最大,其次为捻针角度,再次是捻针频率,最次是捻针时间。Objective: To explore primary secondary factors that influence acupuncture effectiveness and the best combination of acupuncture parameters,in order to provide experimental evidence to elevate acupuncture effectiveness for clinic application. Orthogonal design was adopted to study several factors that influence acupuncture effectiveness. The rats model of kidney- YANG deficiency made by hydrocortisone as the study objects. Methods: 4 influential factors( i. e. selection of acupoints,twisting- rotating angle,twisting-rotating frequency and acupuncture time) plus 3 levels of each factor were arranged to L9( 34) orthogonal table. Treat kidney- YANG deficiency rats by computer controlled acupuncture manipulation device. Serum T and E2 were evaluated by ELISA method and pituitary LH and FSH mRNA expression by real time fluorescent quantitation PCR to illustrate the primary secondary factors and the good bad of different levels in each factor that could affect the acupuncture effectiveness. Results: Among 4 factors mentioned above,the order for influential factors from major to small is acupoints selection,twisting- rotating angle,twisting- rotating frequency and acupuncure time,among which,selection of acupoints was the major factors( P〈0. 05),twisting- rotating angle,twisting- rotating frequency and acupuncture time were the secondary factors( P〈0. 05,P〈0. 05 and P〈0. 05). Conclusion : 1The best acupuncture parameters for treating kidney- YANG deficiency rats were: selecting Shenshu acupoint( B23),90°of twisting- rotating angle,60tpmof twisting- rotating frequency and 60 s of acupuncture time; 2In this study,different acupuncture parameter has different efficacy for kidney- YANG deficiency rats; among them,selection of acupoints became the biggest influential factor,the next was twisting- rotating angle,the secondary was twisting- rotating frequency,and the last one was acupuncture time.
