中性粒细胞弹性蛋白酶及弹性蛋白酶抑制剂在盆底器官脱垂患者中的表达及意义

摘　　要：目的观察盆底器官脱垂患者阴道前壁组织中中性粒细胞弹性蛋白酶(NE)及弹性蛋白酶抑制剂(Elafin)的表达水平。探讨其与盆底器官脱垂的发生关系。方法自2012年4月1日-2013年12月31日该科收治的60例子宫脱垂患者(绝经前26例,绝经后34例),采用real time PCR及Western blot法检测60例子宫脱垂患者(观察组)阴道前壁组织中NE及Elafin的m RNA和蛋白的表达水平,选择同期因妇科良性疾病、无盆底功能障碍性疾病而行子宫全切除术的患者40例(对照组)做比较。结果观察组患者阴道前壁组织中NE的m RNA表达水平为(1.53±0.26),NE蛋白表达水平为(2.08±0.48);Elafin的m RNA表达水平为(1.36±0.25),Elafin蛋白表达水平为(0.84±0.23)。对照组中NE的m RNA表达水平为(0.63±0.22),NE蛋白表达水平为(1.29±0.35);Elafin的m RNA表达水平为(0.57±0.22),Elafin蛋白表达水平为(0.30±0.16)。观察组与对照组比较,差异均有统计学意义(P<0.01),观察组高于对照组。观察组与对照组患者阴道前壁组织中NE及Elafin的m RNA和蛋白表达水平在绝经前、后差异均无统计学意义(P>0.05)。NE与Elafin的表达量呈直线正相关关系(r=0.931、0.782,P<0.05)。结论 NE及Elafin表达的异常可能与盆底器官脱垂的发病有关。Objective To observe the expression levels of neutrophil elastase （NE） and elastase inhibitor （Elafin） in anterior vaginal wall tissue of patients with pelvic organ prolapse （POP）, explore their relationships with POP. Methods From April 1 st, 2012 to December 31 st, 2013, sixty POP patients treated in the hospital were selected as observation group, including 26 premenopausal patients and 34 postmenopausal women, RT-PCR and Western blot were used to detect the expression levels of NE and Elafin mRNA and proteins in anterior vaginal wall tissue of patients in observation group; forty patients receiving total hysterectomy because of gynecological benign diseases and without POP during the same period were selected as control group. Results The expression levels of NE mRNA and protein in observa- tion group were （ 1.53±0. 26 ） and （2. 08±0. 48 ）, respectively; the expression levels of Elafin mRNA and protein in observation group were （1.36±0. 25） and （0. 84±0. 23）, respectively. The expression levels of NE mRNA and protein in control group were （0. 63±0. 22） and （ 1. 29±0. 35 ） , respectively; the expression levels of Elafin mRNA and protein in control group were （0. 57±0. 22） and （0. 30±0. 16）, respectively. There were statistically significant differences between the two groups （P〈0. 01 ） , the expression levels in observation group were higher than those in control group. In the two groups, there was no statistically significant difference in the expression levels of NE and Elafin mRN A and proteins between premenopausal period and postmenopausal period （P〉0.05） . There was a linear positive correlation between NE expression level and Elafin expression level （r=0. 931, P〈0. 05 ; r=0. 782, P〈0. 05） . Conclusion The abnormal expressions of NE and Elafin nmy be correlated with POP.
