摘  要:目的:探讨单纯口服来曲唑(LE)在多囊卵巢综合征患者中促排卵的个体化用药方法。方法:1收集近期不孕症门诊多囊卵巢综合征(PCOS)患者56例、158个促排卵周期。2选择LE启动剂量为5.0mg,无论卵泡生长如何均不添加HMG。3起始用药时间于月经第5天开始,用药天数以5d为准,根据卵巢反应性适度增减,总用药天数不超过10d,服药10d仍无优势卵泡生长则取消本周期。结果:147个周期成功排卵,排卵率93.04%;147个排卵周期中有15个周期是2个成熟卵泡破裂,周期平均排卵数1.10个;排卵前平均子宫内膜厚度9.55mm;38例妊娠,周期妊娠率25.85%;双胎妊娠3例,未出现中度以上卵巢过度刺激综合征(OHSS)。结论:单纯口服来曲唑促排卵,选择合适的启动剂量,根据卵泡发育情况灵活掌握个体化用药天数,能明显提高LE的促排卵效率,避免HMG的使用。Objective:To explore the method of individual application of letrozole for ovulation induction in polycystic ovary syndrome(PCOS).Methods:A total of 56 patients with PCOS(158cycles)were collected.The starting doses of letrozole were designed at 5.0 mg,and human gonadotropin(HMG)was avoided.Letrozole was taken orally on the fifth day of menstruation.Five days were designed as a course of treatment.Then the number of medication days was adjusted appropriately according to ovarian reaction,however,medication wouldn't exceed 10 days.Treatment plan would be cancelled if there was no dominant follicle after 10 days medication of letrozole.Results:All together 147 cycles succeeded,and the success rate was 93.04%.The mean number of ovulation was 1.1,the mean thickness of endometrium was 9.55 mm before ovulation.A total of 38 women got pregnancy,and the pregnancy rate was 25.85%.Three women got twin pregnancy.No moderate or severe ovarian hyperstimulation syndrome(OHSS)occurred.Conclusion:The letrozole at appropriate dose and treatment time according to the follicular development can improve the efficacy of ovulation induction,and avoid application of HMG.
