精准肝切除术在治疗肝胆管结石病中的临床应用

摘　　要：目的研究探讨治疗肝胆管结石患者时应用精准肝切除手术的有效临床意义。方法整群选择2013年5月—2014年5月在该院救治的肝胆管结石患者共120例,将其随机分成两组,定为精准肝切除手术组和非规则肝切除手术组,并对两组进行切除手术,观察两组患者手术时间、术中出血量、结石清除率和患者住院时间的差异以及两组患者术后并发症和不良反应。结果分析两组的手术情况,精准肝切除手术组的手术时间和手术结石清除完全的患者人数都大于非规则肝切除手术组,患者在手术中的出血量和住院时间都大大低于非规则肝切除手术组。两组比较差异有统计学意义(P<0.05)。从不良反应看,精准肝切除手术组患者出现9例不良反应,不良反应率为15.0%;非规则肝切除手术组有20例不良反应,不良反应率为33.3%。两组比较差异有统计学意义(P<0.05)。结论精准肝切除手术在临床治疗肝胆管结石病中有疗效好、治疗时间短、不良反应少等优点,具有重要的临床意义。Objective To study the clinical significance of the application of precise hepatectomy in the treatment for patients with liver and bile duct stones. Methods To select a total of 120 cases of patients accepted the treatment during May 2013 to May 2014 in our hospital, and to get them divided into two groups randomly, as precise hepatectomy group and irregular hepatectomy group, and then to perform surgery for the two groups. Patients were observed with operation time, blood loss difference, stone clearance rate, hospitalization time and postoperative complications and adverse effects from two groups. Results To analyze both groups surgery, operative time and surgical stone clearance of the precise hepatectomy group were greater than the number of ir-regular hepatectomy group, the amount of bleeding during surgery and Hospitalization time were significantly lower than the irreg-ular hepatectomy group. There were significant differences between the two groups （P〈0.05）, with statistical significance. From ad-verse reactions, there were 9 cases with adverse reactions in the precise hepatectomy group, adverse reaction rate was 15.0%; and in the irregular hepatectomy group, there were 20 cases with adverse reactions, adverse reaction rate was 33.3%. Comparison of the two groups was statistically significant （P〈0.05）. Conclusion Precise hepatectomy in the clinical treatment of liver and bile duct stones has advantage of good efficacy, short treatment time, fewer adverse reactions, etc., and it has important clinical significance.
