超声造影和增强CT对微小肝癌的诊断价值

摘　　要：目的探讨超声造影和增强CT对微小肝癌的诊断价值。方法对影像学检查疑为恶性的112例143个≤2.0 cm的肝脏病灶分别行超声造影和增强CT检查,结果与病理对照,比较两种检查技术对微小肝癌的鉴别诊断效能。结果 112例143个病灶中,超声造影和增强CT诊断恶性、良性病变分别为103个、40个和100个、43个。病理诊断恶性病变111个,其中原发性肝细胞肝癌(HCC)79个(高、中、低分化癌分别为19、38、22),肝内胆管细胞癌(ICC)8个(高、中、低分化癌分别为4、3、1),转移性肝癌(MC)24个;良性病变32个。与病理对照,超声造影和增强CT诊断的敏感性、特异性、准确性分别为88.3%、84.4%、87.3%和86.5%、87.5%、86.7%,两者比较差异无统计学意义(P>0.05)。结论超声造影和增强CT诊断微小(≤2.0 cm)肝癌具有相似的效能,超声造影更简便,具有重要的临床应用价值。Objective To investigate the value of contrast-enhanced ultrasound（CEUS） and contrast-en- hanced CT（CECT） in the diagnosis of small liver cancer. Methods One hundred and tweleve patients with 143 small solid liver lesions （ ≤2.0 cm） were examined with contrast-enhanced ultrasound and contrast-enhanced CT, and the diagnoses were confirmed by pathologieal examination. CEUS and CECT findings of 143 small solid liver le- sions in the patients were retrospectively analyzed. Results 111 malignant lesions including 79 primary liver carcino- ma, 8 intrahepatic cholangiocarcinoma and 24 metastatic liver carcinoma and 32 benign lesions were found in 112 pa- tients with 143 lesions. Compared with the results of pathological diagnosis, the sensitivity, specificity and accuracy of CEUS and CECT were 88.3% , 84.4% , 87.3% and 86.5% , 87.5% , 86.7% , respectively, without significant differences between CEUS and CECT in the diagnosis （ P 〉 0.05 ）. Conclusion CEUS and CECT have similar effects in the diagnosis of small liver carcinoma（ ≤2. 0 cm）. CEUS is simpler than CECT in the diagnosis of small liver carcinoma.
