两种维持方法治疗复发性外阴阴道假丝酵母菌病的疗效分析

摘　　要：目的比较观察两种维持方法治疗复发性外阴阴道假丝酵母菌病(RVVC)的疗效,寻找一种疗程短、疗效确切的维持治疗RVVC的治疗方案。方法将112例RVVC患者随机分为观察组和对照组各56例,观察组在维持治疗期间,单纯给予维生素C片阴道上药治疗,疗程3个月;对照组常规内服氟康唑、阴道放置克霉唑栓,疗程6个月。比较两组患者的临床疗效、药物不良反应及复发率。结果观察组和对照组总有效率分别为96.4%和98.2%,两组疗效比较差异无统计学意义(P>0.05);观察组出现阴道胀痛5例(8.9%),对照组13例(23.2%)分别出现恶心、呕吐及阴道灼热感,两组药物不良反应发生率比较差异有统计学意义(P<0.05);观察组1年复发2例(3.6%),对照组复发3例(5.4%),两组复发率比较差异无统计学意义(P>0.05)。结论采用维生素C片阴道上药维持治疗RVVC,具有疗程短、疗效确切、减少口服药物的不良反应、减轻患者经济负担、患者易接受等优点,值得临床推广应用。Objective To observe the therapeutic effects of two kinds of maintenance therapy on treating re- current vulvovaginal candidiasis（RVVC） and to find a maintenance therapic method with shorter course of treatment and better curative effect. Methods One hundred and twelve patients with RVVC were randomly divided into the observation group（ n = 56） and the control group（ n = 56）. The observation group was simply given vitamin C tablets in vagina during maintenance therapy for three months; The control group was treated with oral fluconazol and vaginal suppository of clotrimazole for six months. The clinical efficacy, adverse reactions and recurrence rate were compared between the two groups. Results The total effective rates were 96.4% and 98.2% in the observation group and the control group respectively（ P 〉 0. 05 ） ; Vaginal pain occured in five patients in the observation group （8.9%）, while more adverse reactions including nausea, vomiting and vaginal burning sensation occur in thirteen patients in the con- trol group（23.2% ）, with a significant difference in adverse reaction rate between the two groups （ P 〈 0.05 ）. The disease reoccurred in two cases （3.6%） in the observation group one year after the treatment, while three cases （5.4%） in the control group, without a significant difference between tile two groups（P 〉 0. 05 ）. Conclusion External use of vitamin C tablets in vagina has some advantages of a shorter course, satisfactory curative effect and less drng adverse reactions for the treatment of RVVC.
