血常规中一些指标在2型糖尿病中的新应用

摘　　要：目的探讨血常规中一些指标与2型糖尿病间的关系，试图发现这些指标新的临床应用。方法检测302名2型糖尿病患者的FPG、2h—PG，WBC、RBC、HB、MCV、RDW、Neutrophil（％）、Lymphocyte（％）、NLR。以eGFR（MDRD）分组判断各指标的变化情况；以FPG7．0mmol／L、PPG11．1mmol／L为界限分组，分析与血常规各指标间的关系；并作ROC曲线分析。结果根据eGFR（rMDRD公式）将肾功能分成正常组、轻中度组和重度组，各个指标在肾功能分级上没有变化（P〉0．05），但呈现一定的变化趋势；在FPG分组中RBC、HB有显著差异，MCV有差异，其它指标没有差异；2hPG分组中RBC、HB有显著差异，WBC、RDW、Neutrophil％、NLR有差异，MCV和Lymphocyte％无差异；以PPG7．0mmol／L做上述指标的R0c工作曲线，以Lymphocyte曲线下面积最大为0．897，特异性87％；Lymphocyte%和NLR灵敏度可达100％；特异性在80％以上的依次是RBC、HB、Neutrophil％、WBC、RDW；曲线下面积最大依次为Lymphocyte%0．897、RBC0．831、HB0．775、WBC0．682。结论血常规中的一些指标为糖尿痛心血管疾病和肾病发生和发展提供了又一个便捷的常规监控指标。Objective To investigate the relationship between some indexes of blood routine and type 2 diabetes, trying to find the new clinical application. Methods Detection of 302 type 2 diabetic patients with FPG,2 h-PG,WBC,RBC,HB,MCV,RDW, Neutrophil （%）, Lymphocyte （%）, NLR. Uses eGFR（MDRD） packet to determine the changes of each index; with FPG 7 retool/ L, 2h-PG 11.1 mmol/L as the boundary grouping, and indexes of blood routine analysis of the relationship, analysis ROC curve. Re- sults According to the eGFR （rMDRD formula） to renal function divided into normal group,mild to moderate group and severe group,Each index did not change in renal function classification （P〉0.05）, but has a certain change trend, In an FPG packet, there was significant difference of RBC, HB, MCV were different,no difference in other indexes, 2hPG packet, there was significant difference of RBC, HB, WBC, RDW, Neutrophil~, NLR have differences, and no difference in MCV and Lymphocyte% ; PPG7. 0 mmol/L to do ROC work curve of the index, with the area under the Lymphoeyte% curve for a maximum of 0. 897, specificity of 87 % ;Lymphocyte% and NLR sensitivity of up to 100% ;specificity above 80% followed by RBC, HB, Neutrophil%, WBC, RDW; the area under the curve of maximum followed by Lymphocyte% 0. 897,RBC 0. 831, HB 0. 775,WBC 0. 682. Conclusion Some in dexes of blood routine for diabetic cardiovascular and kidney disease occurrence and development provides a convenient routine mo nitoring index.
