基底细胞样乳腺癌临床病理分析

摘　　要：目的观察基底细胞样乳腺癌的病理形态学及免疫组织化学表达，探讨其诊断依据。方法筛选分析308例乳腺癌，根据免疫表型分为腺腔A型、腺腔B型、正常乳腺型、HER2过表达型、基底细胞样乳腺癌进行临床病理分析、形态学观察及免疫组织化学染色。结果基底细胞样乳腺癌共51例16．6％（51／308），病理形态学特征主要有：肿瘤细胞巢团状排列及合体状生长，缺乏腺管结构；出现不同程度坏死区域，可有地图样坏死；推挤性浸润边缘；间质显著淋巴细胞浸润；肿瘤细胞异型明显，核分裂指数高。结论基底细胞样乳腺癌具有独特的形态学特征和免疫表型。Objective basal cell pathological morphology and immunohistochemical expression of the basal-like subtype of inva- sive breast carcinoma（BLBC） ,and to discuss the diagnosis basis. Methods Screening analysis of 308 cases of breast cancer,Accord- ing to the immune phenotype is divided into luminal A, luminal B, null subtypes, H ER2-overexpressing and basal-like type and clini- cal pathological analysis,morphoiogica[ observation and immunobistochemical staining. Results Among the breast cancer patients, the incidence of BLBC was 16.6% （51/308）. Pathological morphological features included nest structure,are fit to grow,the lack of the structure of the gland first,appear different degree of necrotic area, push the edge of infiltration,interstitial lymphocytic infil- trates significantly,tumor cells different apparent,fission as high. Conclusion BLBC showed special morphologic and immunophenotypic features.
