颈椎前路植入PEEK PREVAIL系统手术方式治疗单节段颈椎病  

Comparison of the PEEK PREVAIL System and the Traditional Anterior Cervical Anterior Decompression and Fusion Surgery in the Treatment of Single Segment Cervical Spondylosis

摘　　要：目的本研究旨在探讨颈椎前路植入PEEK PREVAIL系统手术方式与传统前路减压植骨融合术治疗单节段颈椎病的疗效差异。方法对本院2011年3月至2015年3月收治的67例单节段颈椎病患者的临床资料进行回顾性分析,根据手术方式分为PEEK PREVAIL系统组35例(前路植入PEEK PREVAIL系统治疗)、传统组32例(前路减压植骨融合术治疗),对两组患者的手术情况及术后随访资料进行统计分析。结果 PEEK PREVAIL系统组的手术时间、术中出血量、住院时间均显著低于传统组患者(P<0.05)。PEEK PREVAIL系统组与传统组患者的手术前后日本骨科协会(Japanese orthopaedic association,JOA)、颈椎残障功能指数(neck disability index,NDI)、视觉模拟评分(visual analogue scale,VAS)评分组间比较差异均无统计学意义(P>0.05),术后6个月两组患者的JOA、NDI、VAS评分较术前均显著降低(P<0.05)。PEEK PREVAIL系统组患者的Odom优良率为91.43%,低于传统组患者的93.75%,但差异无统计学意义(P>0.05)。PEEK PREVAIL系统组术后6个月融合率达到100%,而传统组融合率为96.9%,差异无统计学意义(P>0.05)。PEEK PREVAIL系统组患者的并发症发生率为11.43%,低于传统组患者的46.88%且差异具有统计学意义(P<0.05)。结论颈椎前路植入PEEK PREVAIL系统手术较传统前路减压植骨融合术治疗单节段颈椎病具有手术时间短、出血量少、住院时间短、术后并发症低的优点,且手术效果基本相当。Objective To explore the effect of anterior cervical implant PEEK PREVAIL system and traditional anterior cervical decompression and bone graft fusion in the treatment of single segment cervical spondylosis. Methods The clinical data of 67 patients with single segment cervical spondylosis patients admitted to our hospital were divided into PEEK PREVAIL group（35 cases）and the traditional group（32 cases）,and were retrospectively analyzed. Results The operation time,amount of bleeding,hospitalization time in the PEEK PREVAIL group,were significantly lower than the traditional group（P 〈 0. 05）. The PEEK PREVAIL group and the traditional group had no difference in JOA,NDI,VAS score（P 〉 0. 05）. 6 months after op-eration,JOA,NDI,VAS score was lower than that before operation in both group（P 〈 0. 05）significantly. PEEK PREVAIL group of patients＇excellent and good rate was 91. 43% ,which was lower than that of the traditional group 93. 75% ,but the difference was not statistically significant（P 〉 0. 05）. 6 months after operation,PEEK PREVAIL system group of the fusion rate was 100% ,and the traditional group of the fusion rate was 96. 9% . There was no statistically significant difference（P 〉 0. 05）. PEEKPREVAIL group of patients with complications was 1 1 . 4 3 % ,which was lower than that of the traditional group were 46. 88% and the difference was statistically significant（P 〈 0. 05）. Conclusion Compared with the traditional PEEK PRE-VAIL system,anterior cervical operation with anterior decompression and fusion for single segment cervical spondylosis has the advantage of shorter operation time,less bleeding,shorter hospital stay,lesspostoperative complications were low.
