补阳还五汤加麝香治疗脊髓损伤临床研究

摘　　要：目的:观察补阳还五汤加味麝香治疗脊髓损伤的疗效。方法:65例随机分为治疗组一21例、治疗组二23例和对照组21例;所有病例均给予脊髓损伤常规治疗,治疗组一给予补阳还五汤加味麝香治疗,治疗组二给予补阳还五汤治疗,对照组给予大活络丸治疗。分别于治疗后1个月、3个月、12个月,对神经感觉、肌力、日常生活能力按国际脊髓损伤神经分类标准(ASIA)进行评分,并利用肌电图对腓总神经和胫神经的传导功能进行测定。结果:感觉指数评分,治疗后1个月、3个月、12个月,治疗组一和治疗组二均较对照组有明显升高(P<0.05),且在治疗后3个月和12个月时,治疗组一的评分较治疗组二升高更为明显(P<0.05)。肌力指数评分、日常生活能力评分、腓总神经和胫神经的传导功能比较,治疗组一在治疗后3个月、12个月时较对照组有明显升高(P<0.05);结论:补阳还五汤有一定促进脊髓损伤功能恢复的作用,加麝香后治疗脊髓损伤作用显著提高。Objective： To observe the chnic effect of Buyang Huanwu Tang plus musk to improve the treatment of spinal cord injury and to evaluate the therapeutical effect of musk in this remedy. Method： 65 patients with spinal cord injury were assigned randomly into trial group I with 21 cases, trial group II with 23 cases and controlled group with 21 cases. Trial group Ⅰ and Ⅱ： remedy of Buyang Huanwu Tang plus musk and Buyang Huanwu Tang were given respectively beside the conventional therapy of spinal cord injury at the dosage of 3 times a day for 4 weeks. These remedies were given at the 3rd day when the patient was hospitalized. For the control group, Da Huoluo Wan was given instead of Buyang Huanwu Tang plus musk or Buyang Huanwu Tang in Trial group Ⅰ and Ⅱ. Grading of nerve function of sensing, muscle strength, activities of daily living were implemented according to the standard of ASIA along with the determine the conduction of common peroneal and tibial nerve by electromyogram before and 1, 3, 12 months after the 4-week-treatment. Result： For the nerve sensory function, the trial group Ⅰ and Ⅱwere better than the control group 1, 3 and 12 months after treatment （ P〈0.05 ） while the trial group Ⅰ was better than the trial group Ⅱ3 and 12 months after treatment （ P〈0.05 ） .And for the muscle strength, activities of daily living and the conduction of common peroneal and tibial nerve, trial group Ⅰ was better than that of the control group 3 and 12 months after treatment （P〈0.05） ; but for the trial group Ⅱ, though these values were higher than the control group, the differences were of no significance （ P〉0.05 ） .Conclusion： the remedy ofBuyang Huanwu Tang plus musk was helpful in the treatment ofspinai cord injury with musk functioned importantly in this remedy. So, musk have clinical significance as the Du channel meridian-lead-in drug. It is worth researching further the mechanisms of this remedy to improve the treatment of spinal cord injury.
