血清TNF-α、IL-6水平对膝骨关节炎的诊断价值及其相关性研究

摘　　要：目的研究血清TNF-α、IL-6水平对膝骨关节炎的诊断价值及与病变程度的相关性。方法选取2012年3月至2015年3月在我科诊治的65例单膝骨关节炎患者作为研究组,根据病变程度分为早中晚期,设立为早期组、中期组和晚期组,选择同期健康体检者65例作为对照组,所有入选者抽取静脉血,比较各组血清中TNF-α和IL-6水平。结果研究组患者血清中TNF-α、IL-6水平明显高于对照组(P<0.05),TNF-α水平随着病情进展有上升趋势,晚期组患者血清中TNF-α水平高于早期组与中期组且差异均有统计学意义(P<0.05);中期组患者血清中IL-6水平高于早期组(P<0.05),病情加重至晚期时IL-6水平下降,晚期组患者血清中IL-6水平低于中期组(P<0.05)。结论 KOA患者血清TNF-α、IL-6水平较健康者明显上升,且与病情严重程度有一定相关性。Objective To study the value of serum TNF-α and IL-6 in the diagnosis of knee osteoarthritis and its correlation with the severity of the disease. Methods 65 patients with single knee osteoarthritis treated in our department from March 2012 to March 2015 were selected as the research group. According to the extent of the lesion,the early group,middle group and late group were set up and 65 healthy people were selected as the control group. The serum levels of TNF-and IL-6 in patients and healthy controls were detected. Results The serum concentration of TNF alpha and IL-6 in patients were significantly higher than that of the controls（ P 〈0. 05）. The TNF-a concentration was increased with the progression of the disease. The serum TNF-α level in late group patients was higher than that of early and medium group patients（ P 〈0. 05）. The serum IL-6 level in medium group patients was higher than that of early group patients（ P 〈0. 05）. The serum IL-6 levels were decreased as the illness aggravating to late stage. The serum IL-6 level in late group patients was lower than that of medium group patients（ P 〈0. 05）. Conclusion The serum TNF-α and IL-6 levels in KOA patients were significantly higher than those of healthy controls,and it has a certain correlation with the severity of the disease.
