摘  要:目的研究清胰醒脑颗粒对肝郁气滞与食积湿滞型胰性脑病(pancreatic encephalopathy,PE)大鼠脑保护作用的疗效对比,并探讨机制。方法采用经大鼠胰管注入牛磺胆酸钠并尾静脉注射精制猪磷脂酶A2(phospholipase A2,PLA2)的方法建立PE模型,肝郁气滞组加用慢性束缚的方法,食积湿滞组予过量喂食高蛋白、高热量饲料的方法,肝郁气滞治疗组和食积湿滞治疗组分别在肝郁气滞组和食积湿滞组的基础上,予中药清胰醒脑颗粒灌胃治疗。观察各组大鼠的一般情况,7 d后检测大鼠脑组织含水量,qRT-PCR检测脑组织中缺氧诱导因子-1α(hypoxia inducible factor-1α,HIF-1α)mRNA的表达的结果。结果肝郁气滞治疗组的一般情况、脑组织含水量和HIF-1αmRNA的表达量等均优于肝郁气滞组,食积湿滞治疗组上述情况分别优于食积湿滞组(P<0.05),而两种证候的模型组以及治疗组组间比较,差异均无统计学意义(P>0.05)。结论清胰醒脑颗粒治疗PE疗效确切,而针对肝郁气滞与食积湿滞两种证型,治疗无明显特异性,其作用可能与降低脑损伤时脑组织含水量,提高HIF-1αmRNA的表达量有关。Objective To compare the effect of Qingyi Xingnao between the Liver depression and Qi stagnation of pancreatic en- cephalopathy in rats and investigate the mechanism. Methods The PE models were all induced by retrograde infusion of 4.0 % sodium taurocholate into the pancreatobiliary duct and by infusion phospholipase A2 (PLA2) into the caudal vein. The rats of the Liver depression group were made by chronic bondage, the Qi stagnation group by fed high protein and high quantity of heat of feed additionally. And all the treatment groups were all fed by Qingyi Xingnao. Their general situation was observed. After seven days, the brain water content of all groups were measured using the method of dry - wet weight. And the hypoxia inducible factor -1α( HIF - 1α) mRNA expression changes of each group in brain tissue were detected by qRT - PCR. Results The water con- tent in brain tissue homogenate, and HE dyeing of pancreas and brain tissue, and HIF - 1α mRNA expression in brain tissue of treatment groups were all superior to the model groups( P 〈 0.05 ). And that between the Liver depression and Qi stagnation of PE has no difference (P 〉 0.05 ). Conclusion The protective effect of Qingyi Xingnao is obvious for cerebral protection of PE rats. And that between the Liver depression and Qi stagnation of PE has no difference. Its mechanism may be related to restrain enceph- aledema and upregulate the expression of HIF - 1α mRNA in brain tissue.
