摘 要:目的研究并总结多层螺旋CT重建技术应用于骨关节创伤中的临床效果和实际意义。方法抽取骨科2010年3月至2015年3月收治踝关节受伤骨折患者93例为实验研究对象。所有患者经影像学检查等确诊踝骨骨折。所有研究对象进行透视及CT三维重建,比较2种检测方法术前分型评价与手术确诊符合率,术后康复质量以及内固定质量评分。结果CT三维重建技术术前检测确认例数与手术确认结果符合度高于透视技术,CT三维重建技术Kappa值、灵敏度以及特异性均优于透视技术。在旋前外展型、旋前外旋型和旋后外旋型检测中,CT三维重建检出解剖学复位高于透视技术(P<0.05),在旋前外旋型和旋后外旋型检测中,CT三维重建检出内固定适宜高于透视技术(P<0.05)。结论多层螺旋CT重建技术在关节创伤中的应用价值高,能够准确判断骨折类型、手术入路和治疗方案。Objective To study and summarize the clinical effects and the practical significance with multislice CT reconstruction technology in treating joint trauma. Methods 93 cases with injured ankle fracture treated in our hospital from March 2010-March 2015 were selected as experimental subjects. All patients were diagnosed with imaging examination as ankle fractures. All the subjects were undergone roentgenoscopy and CT three- dimensional reconstruction examination, the classification evaluation of the 2 approaches and the surgical diagnosis coincidence rate, postoperative recovery quality and internal fixation of quality score were compared. Results The results showed that the coincidence rate of CT three-dimensional reconstruction confirmed cases with surgery was higher than that of roentgenoscopy, Kappa value, sensitivity and specificity of CT three- dimensional reconstruction were superior to roentgenoscopy technique. Before turning outreach, outside pronation and supination out type tests, CT three dimensional reconstruction detection anatomy reset was higher than that of roentgenoscopy technique (P〈0.05), type out in front of the spin and spin after spin type tests, suitable internal fixation determined with CT three dimensional reconstruction detection was higher than that of roentgenoscopy technique (P〈0.05). Conclusion The application value of multislice CT reconstruction technology in examining joint trauma is high, which can accurately determine fracture type, surgical approachand treatment plan.
