腰大池腹腔分流术在新型隐球菌性脑膜炎中的应用

摘　　要：目的观察腰大池腹腔分流术治疗新型隐球菌性脑膜炎(CNM)合并脑积水的临床疗效。方法选择2009-02-2014-05于我院神经外科就诊并被诊断为新型隐球菌性脑膜炎的患者52例,按照随机化的原则将入选患者分为对照组和研究组各26例,研究组采用腰大池腹腔分流术治疗,对照组采用常规保守治疗,治疗后进行疗效对比。结果研究组显著好转24例,其中脑积水明显缓解20例,脑室无明显改善4例,未好转2例,有效率为92.31%;对照组显著好转17例,其中脑积水明显缓解10例,脑室无明显改善7例,未好转9例,有效率为65.38%,2组比较差异有统计学意义(P<0.05)。结论腰大池腹腔分流术的操作较为简便,迅速快捷,对新型隐球菌性脑膜炎合并脑积水的疗效显著。Objective To observe the clinical efficacy of lumbar cisterna combined with peritoneal shunt in cryptococcus neoformans meningitis （CNM ） patients complicated with hydrocephalus.Methods 52 cases with CNM from February 2009 to May 2014 in neurosurgery department of our hospital were selected and randomly divided into the control group and study group ,26 cases in each group. Patients in study group were treated with lumbar cisterna combined with peritoneal shunt ,and the control group received conservative treatment ,then we compared the efficacy of two groups.Results 24 cases in study group improved significantly with clinical effective rate of 92.31% including obviously improvement in 20 cases with hydroceph‐alus ,cerebral ventricle with no significant changes in 4 cases and no improvement in 2 cases.Meanwhile ,17 cases in the control group showed remarkable improvement with the effective rate of 65.38% including obviously improvement in 10 cases with hy‐drocephalus ,cerebral ventricle with no significant changes in 7 cases and no improvement in 9 cases ,and there was statistical difference between two groups （P〈0.05）.Conclusion The operation of lumbar cisterna combined with peritoneal shunt is sim‐ple and efficient ,which has an considerable effect on cryptococcus neoformans meningitis （CNM） patients complicated with hy‐drocephalus.
