摘　　要：目的探讨严重盆骨骨折为主的多发伤应用DCS(应用损伤控制外科)的可行性,总结其救治体会。方法随机抽取该院2010年11月—2014年11月收治的严重盆骨骨折为主的多发伤患者40例,采用回顾性分析的方法 ,观察其诊治过程、死亡率、后遗症等。结果严重盆骨骨折为主的多发伤患者入院之后,即可进行伤情判断以及抗休克治疗,控制伤处的出血情况,入ICU(重症监护室)进行抢救及监护治疗,待患者全身情况稳定后再施予确定性手术,经统计分析,严重盆骨骨折为主的多发伤40例伤者当中,4例死亡,3例死亡伤者的死因为失血过多而死,1例死于术后感染。伤者的总存活率为90.00%。结论在对严重盆骨骨折为主的多发伤患者治疗过程当中,应用DCS的救治理念与手段,在明确患者伤情的基础上,积极给予控制出血手术,可避免患者因出血过多而导致缺血缺氧,损害重要的脏器,提高患者的存活率,此为救治之关键所在。Objective To study the feasibility of DCS in the treatment of polytrauma patients with severe pelvic fractures and summarize the treatment experience.Methods 40 polytrauma patients with severe pelvic fractures admitted to our hospital between November 2010 and November 2014 were randomly selected and the treatment process,mortality and sequelae were retrospectively analyzed.Results Severe pelvic fracture of multiple trauma patients admitted to hospital after the injury situation judgment and anti-shock treatment,can be made to control the wound bleeding,into the ICU（intensive care unit） to rescue and care treatment,for patients with deterministic operation,the whole body is in a stable condition after giving by the statistical analysis,patient survival rate was 90.00%.Conlusion In the treatment of polytrauma patients with severe pelvic fractures,the application of DCS treatment idea and means,on the basis of clear patient injury,by controlling bleeding through operation actively,can avoid ischemia hypoxia and damage of important organs caused by massive bleeding and improve the survival rate.It is the key of treatment.
