包皮环切术与包皮环套术的对比研究

摘　　要：目的 研究传统包皮环切术、包皮环套术治疗包皮过长的临床疗效.方法 对320例包皮过长的手术患者随机分为包皮环切术（A组）、包皮环套术（B组）二组,每组160例,分别行传统包皮环切术、包皮环套术治疗.回顾分析并比较各组术后一般情况及并发症,对所测指标进行统计学分析.结果 传统包皮环切术、包皮环套术治疗包皮过长均有较好的近期疗效.当患者年龄＜ 14岁时,B组手术时间、术中出血、瘢痕宽度均小于A组（P＜0.05）.两者在术后包皮重度水肿、术后继发出血、术后疼痛药物使用等术后并发症发生率无明显差别（P＞0.05）;当患者年龄＞14岁时,A组重度包皮水肿发生率、术后伤口裂开发生率、继发出血发生率、再次手术发生率、术后止痛药物使用率、伤口愈合时间均明显低于B组（P＜0.05）.结论 当患者年龄＜14岁时首选包皮环套术,它具有手术时间更短、操作更简单、术后护理更方便的有点;当患者年龄＞14岁时首选传统包皮环切术,手术费用更低、术后并发症更少、更安全。Objectives To study the traditional circumcision,the foreskin collar prepuce Clinical efficacy of treatment.Methods 320 cases of surgical patients were randomly divided prepuce circumcision （A group）,circumcision sleeve surgery （group B）,two groups of 160 patients in each group were performed traditional circumcision,the foreskin collar treatment.Retrospectively analyzed and compared in each group after the general situation and complications of the measured indicators were statistically analyzed.Results Traditional circumcision,the foreskin collar prepuce term efficacy in the treatment have better.When patients younger than 14 years old,group B operative time,blood loss,scar width was less than in group A （P 〈 0.05）.Both foreskin postoperative severe edema secondary to postoperative bleeding,postoperative pain medication use,such as the incidence of postoperative complications was no significant difference （P 〉 0.05）;when patients older than 14 years old,A group of severe edema foreskin rate,incidence of postoperative wound dehiscence,the incidence of secondary hemorrhage,the incidence of reoperation,postoperative pain medication usage,wound healing time was significantly lower than in group B （P 〈0.05）.Conclusions When patients younger than 14 years old foreskin collar preferred technique,which has a shorter operative time,more simple operation,postoperative care a little more convenient;patients older than 14 years old when the first choice of traditional circumcision,it surgery lower cost,fewer complications,and more secure.
