摘  要:目的分析腹腔内压力升高对血管外胸腔积液形成的影响及可能的相关机制。方法连续选择2013年10月~2014年10月人笔者医院考虑诊断为腹腔内压力升高患者共95例为研究对象,该研究取得笔者医院伦理委员会通过及患者、家属的知情同意权后,按照腹腔内压力高低将其分为3组,低压力组(12cmH2O≤IAP〈15cmH2O,n=44例)组,中压力组(15emH2O≤IAP〈25cmH2O,n=30例)和高压力组(IAP≥25cm2HO,n=25例),对比分析3组患者的胸腔积液量及相关的生化指标的差异性。结果中高压力组患者在相同时间段的胸腔积液量均显著高于低压力组,差异均有统计学意义(P〈0.05);且各组的胸腔积液量随着时间延长均升高,第4天的胸腔积液量显著高于第1天,差异均有统计学意义(P〈0.05)。中高压力组患者的胸腔积液LDH含量均显著高于低压力组,而pH值和葡萄糖含量均显著低于低压力组,差异均有统计学意义(P〈0.05)。结论腹腔内压力升高可使胸腔积液生成量增加,且胸腔积液生化指标的异常与腹腔内压力相关。Objective To study the effect and mechanism of intraperitoneal pressure on extravascular effusion. Methods A total of 95 consecutives for the experiment from Octomber 2013 to Octomber 2014 were individed into the low pressure group( 12cmH2O≤ IAP 〈 15cmH2O,n =44) ,the median pressure group(15cmH2O≤IAP 〈25cmH2O,n =30)and the high pressure group(IAP≥25cmH2O,n = 25) ,then to compare the difference of extravascular effusion and the biochemical index. Results The same periods of extravascular effu- sion in the median and high pressure groups were significantly higher than the low pressure group(P 〈0.05) and the volume of extravas- cular effusion was increasing with time prolong( P 〈 0.05 ). The levels of LDH in the extravascular effusion of the median and high pressure groups was significantly higher , pH value and glucose level were significantly lower than the low pressure group(P 〈 0.05). Conclusion The volume of extravascular effusion could be increasing with the intraperitoneal pressure up. The biochemical index changes related with the intraperitoneal pressure.
