摘 要:目的探讨骨性关节炎(OA)患者血清中的骨代谢指标的变化情况及其机制。方法选取60例健康正常人作为正常对照组,将98例OA患者分两组,即轻度患病组(Ⅰ级+Ⅱ级)43例,重度患病组(Ⅲ级+Ⅳ级)55例,用电化学发光免疫技术检测3组患者血清中(25-OH)维生素D、骨钙素(Osteoc)、β-胶原特殊序列(β-Cross)和甲状旁腺素(PTH)的浓度。结果血清骨代谢指标中,(25-OH)维生素D和Osteoc在OA发病时均显著降低,其中Osteoc在重度患病组表现更显著。OA患病组与正常对照组比较β-Cross差异无统计学意义。PTH在OA发病时升高(P<0.01),且随着病程的加重而增高。结论在OA患者中血清(25-OH)维生素D和Osteoc水平明显下降,促进了PTH的分泌,可能影响了骨生成和骨转化过程,促进了OA的发生。Objective To discuss the changes of bone metabolism and the mechanism in the osteoarthritis. Methods The 25-hydroxyvitamin D status,Osteoc,β-Cross and PTH in serum were measured in 60 healthy individuals( control group),98 osteoarthritis patients( mild illness OA group( Ⅰ+Ⅱ grade level,43 cases) and severe illness group( Ⅲ+Ⅳ grade level,55 cases) by electro chemiluminescence immunoassay technology. Results During OA attack,PTH and Osteoc were decreased significantly,especially for the Osteoc in the severe illness group. The content of β-Cross was no difference between the OA group and control group. At the onset of OA,the PTH was increased( P〈0. 01),and increased anymore as the OA worsen. Conclusion The levels of( 25-OH) vitamin D and Osteoc were significantly decreased,which promoted the secretion of PTH,may probably affected the process of produce and changes of bone,and promoted OA.
