摘  要:目的探讨磁共振不同高b值扩散加权成像(DWI)在早期缺血性脑梗死患者诊断中的应用价值。方法分析32例急性期、亚急性期缺血性脑梗死患者的资料,3.0TMR上行常规MR序列及DWI(取b值=1 000 s/mm2、b值=3 000 s/mm2)扫描,测量患者梗死区(病灶组)和对侧相应正常区域(对照组)的表观扩散系数(ADC),并计算不同b值下的相对ADC值(r ADC)。结果随着b值的增加,病灶显示更清楚,且能较敏感地发现较小的病灶;随着b值的增加,病灶组和对照组的ADC值均降低,差异均有统计学意义(P<0.05);不同b值下两组r ADC值相比差异无统计学意义(P>0.05);同一b值下病灶组的ADC值较对照组降低,差异有统计学意义(P<0.05)。结论与常规MRI相比,DWI对早期脑梗死具有较高的诊断应用价值,高b值能较敏感地发现小病灶。Objective To evaluate the value of different higher b value magnetic resonance imaging technology on the application of early diagnosis of ischemic cerebral infarction. Methods The data of 32 patients with acute and sub-acute cerebral infarction were analyzed,the conventional MR sequence and DWI sequence were played on 3.0T,the apparent diffusion coefficient( ADC) of the cerebral infarction and the contralateral normal area were measured and the relative ADC were calculated. Results With the increase of b value,the lesions were more clear and it is easier to found the smaller lesions,and the ADC values of lesion group and the control group were decrease significantly( P〈0. 05). With the same b value,the ADC values of lesion group was lower than the one of control group significantly( P〈0. 05). Conclusion Compared with conventional MRI,DWI has a high diagnostic value for early ischemic cerebral infarction,and the higher b value could be more sensitive to smaller lesions.
