脂蛋白相关磷脂酶A2与肾病综合征血栓形成的相关研究

摘　　要：目的:探讨脂蛋白相关磷脂酶A2(Lp-PLA2)与肾病综合征(NS)血栓形成的相关性。方法 :回顾性分析2009年8月至2014年12月入住我院的720例NS患者的临床资料,其中合并血栓组65例,未合并血栓组655例。检测患者血清Lp-PLA2水平。分别通过Pearson单因素与多元Logistic回归分析的方法对Lp-PLA2与NS血栓形成的相关性进行分析。结果:①经Pearson单因素分析,无血栓组与血栓组患者在尿素氮、D-二聚体、血小板及Lp-PLA2水平差异均具有统计学意义(P<0.05或P<0.01)。②经多元Logistic回归分析,血小板、D-二聚体及Lp-PLA2为NS并发血栓的危险因素(均P<0.05)。③采用非条件Logistic回归模型比较不同水平Lp-PLA2与患者的血栓发生风险的相关性,模型1结果显示,血栓发生风险随着Lp-PLA2水平的上升而逐渐增加,比值比(OR)值分别为1.000、1.778、2.490、19.187;模型2结果显示,OR值分别为1.000、1.209、1.601、9.238。结论:NS患者血清Lp-PLA2水平升高可能成为预测血栓发生的独立预测因素,对NS合并血栓具有一定的预测价值。Objective To investigate the correlation of lipoprotein-associated phospholipase A2（Lp-PLA2） with thrombosis in nephrotic syndrome（NS）. Methods The clinical data of 720 cases of NS patients admitted from August 2009 to December 2014 were retrospectively analyzed,including 65 cases with thrombosis,and 655 cases without thrombosis.Serum Lp-PLA2 levels were detected in patients. Pearson univariate analysis and Logistic regression analysis were used to detect the correlation between Lp-PLA2 and thrombosis in NS. Results 1By Pearson univariate analysis,statistically significant differences in urea,D-dimer,platelet and Lp-PLA2 level were found between thrombosis and non-thrombosis groups（P 0.05 or 0.01）; 2 By Logistic regression analysis,the risk factors for NS complicated by thrombosis were：platelets,D-dimer and Lp-PLA2（all P0.05）; 3 Non-conditional Logistic regression model assessing the correlation between levels of Lp-PLA2 with risk of thrombosis,Model 1 showed the risk of thrombosis gradually increased with the rising of Lp-PLA2 level,odds ratio（OR） were 1.000,1.778,2.490,19.187; Model 2 showed the OR values were 1.000,1.209,1.601,9.238. Conclusions The increase of serum level of Lp-PLA2 in patients with NS might be an independent predictor of thrombosisand has some predictive value for thrombosis.
