手法复位结合外科脱位入路技术治疗股骨头骨折疗效观察

摘　　要：目的：探讨外科脱位入路技术治疗股骨头骨折的临床疗效。方法：选取髋臼后壁骨折合并股骨头骨折患者12例,采用外科脱位入路行切开复位内固定治疗,术后随访观察切口愈合、骨折愈合、患肢功能恢复及并发症的发生情况。结果：患者手术切口均甲级愈合,2-4个月后骨折均愈合,平均愈合时间为2.5个月。所有患者均获得随访,随访时间15-20个月,平均随访时间为15.7个月。依据Fhompson-Epstein评分评价疗效,优7例、良3例、可1例、差l例。术后并发股骨头缺血性坏死1例,经口服股骨头坏死愈胶囊及养血止痛丸治疗,病情得到控制;行人工全髋关节置换术1例。无感染、异位骨化、髋关节再脱位等并发症发生。结论：采用外科脱位入路技术治疗髋臼后壁合并股骨头骨折,能使骨折端达到解剖复位,使患者早期恢复患肢髋关节功能活动,减少创伤性关节炎和股骨头坏死发生。Objective：To evaluate the clinical efficacy of surgical dislocation approach technology in the treatment of femoral fractures.Methods：12cases in our department with surgical dislocation of the technical approach open reduction and internal fixation of posterior wall acetabular fracture associated with femoral fractures,were followed up wound healing,fracture healing,limb functional recovery and occurrence of complications.Results：The incision healed well,two to four months after the fracture healed with a median 2.5months,all the patients were followed up for 15 to 20months,median 15.7months.Fhompson-Epstein score based on evaluation of efficacy,excellent in 7cases,good in 3cases,1case and poor l cases.Postoperative avascular necrosis one case,oral capsule and femoral head necrosis more pain pills nourishing treatment,the condition under control;artificial total hip arthroplasty in 1case.No infection,heterotopic ossification,hip dislocation and other complications occur again.Conclusion：The surgical approach in the treatment of dislocation posterior wall acetabular femoral head fracture,can fracture anatomical reduction and enable the patient to the early recovery of limb function of hip joint activities to reduce traumatic arthritis and femoral head necrosis.
