支架球囊辅助栓塞椎基底动脉瘤

摘　　要：目的对困难椎基底动脉瘤,探讨特殊的支架球囊辅助治疗技术的可行性。方法回顾性总结了该院在近2010年1月—2013年11月收治的椎基底动脉瘤共64例。按照治疗方法不同随机分为单纯栓塞组(31例)和支架球囊组(33例)。前者只使用栓塞术,后者在栓塞术基础上使用支架球囊。观察两组取得的疗效。结果支架球囊栓塞组33例,1例患者死亡,32例患者恢复好,出院后1年随访,30例患者生存情况稳定,其中1例复发;单纯栓塞组31例,1例患者死亡,30例患者恢复好,出院后一年随访,28例患者生存情况稳定,其中3例复发。术后,两组大部分栓塞率、瘤颈残留率、完全栓塞率比较差异无统计学意义(P>0.05);随访结果显示支架球囊组完全栓塞率高于单纯栓塞组,差异有统计学意义(P<0.05)。结论对于困难的椎基底动脉瘤的栓塞,支架、球囊等辅助技术栓塞是可行的。Objective To discuss feasibility of balloon/stent assist technique for treating complex vertibrobasilar aneurysms. Methods Summarized retrospectively in our department between January 2010 and November 2013 64 cases with vertibrobasilar aneurysms were treated. According to the different treatments were randomly divided into simple embolization group（31 cases） balloon and stent group（33 cases）. The former only use embolization and the latter on the basis of embolization use stents balloon.Observe two groups of curative effect. Results Stent balloon embolization group of 33 cases, 1 case patients died, restore good, 32 patients discharged from hospital after one year follow-up, 30 patients survival in a stable condition, including 1 case of recurrence; Simple embolization group of 31 cases, 1 patient died, restore good, 30 patients discharged from hospital after one year follow-up, 28 patients survival in a stable condition, including 3 cases of recurrence. After surgery, two groups of most embolism rate, tumor neck residual rate, complete embolism rate comparison difference was not obvious,P〉0.05; Follow-up results showed that the stent balloon complete embolism rate higher than pure embolization group,P〈0.05, the difference was statistically significant. Conclusion To complex vertibrobasilar aneurysms, balloon/stent assist technique is safe and feasible.
