摘 要:目的检测糖尿病肾病患者血浆内皮微粒(EMP)、抗内皮细胞抗体(AECA)等相关指标的变化并探讨其临床诊断价值。方法应用流式细胞术检测血浆中EMP的水平,ELISA方法检测血浆AECA、血小板α-颗粒膜蛋白(GMP-140)和Von Willebrand因子(简称v WF)水平,并运用受试者工作特征(ROC)曲线分析其诊断价值。结果糖尿病肾病患者血浆EMP、AECA、GMP-140和v WF水平的检测水平明显高于正常对照组,且差异有统计学意义(P<0.01)。也较非糖尿病肾病组升高。EMP、AECA、GMP-140和v WF在ROC曲线下面积分别为0.940、0.993、0.623和0.680。结论糖尿病肾病患者血浆EMP、AECA、GMP-140和v WF水平明显增高,EMP和AECA的敏感性和特异性均较高,对诊断具有一定的意义。Objective To detect the level of plasma endothelial microparticles and anti-endothelial cell antibodies in patient with diabetic nephropathy and investigate the clinical diagnostic value. Methods The levels of endothelial microparticles,anti-endothelial cell antibodies, GMP-140 and v WF in plasma were measured by flow cytometry and ELISA. The receiver operating characteristic curve(ROC) was used to analyze the diagnostic value. Results The levels of endothelial microparticles, anti-endothelial cell antibodies and GMP-140 and v WF in plasma were higher than those of the control group(P〈0.01).The areas under the ROC curve of EMP, AECA, GMP-140 and v WF were 0.940, 0.993,0.623 and 0.680, respectively. Conclusion The levels of endothelial microparticles, AECA, GMP-140 and v WF in plasma were obviously higher in patient with diabetic nephropathy.The sensitivity and specificity of EMP and AECAwere high, and could be used for the diagnosis.
