对食管癌术后胸胃排空障碍观察与护理的临床研究

摘　　要：目的探讨食管癌术后胸胃排空障碍的临床特点、病因,并提出治疗和护理措施。方法回顾性分析2011年8月—2013年8月期间在该院行食管癌切除术后出现胸胃排空障碍的60例患者临床资料,均为功能性排空障碍患者。按照护理方式分为观察组和对照组,对照组行常规的排空障碍护理方法,观察组在此基础上配合中医针灸调理,比较2组术后排空情况及临床护理效果。结果对照组30例患者经护理干预后,28例痊愈,2例恶化再次手术。15例于术后1个月恢复胃肠动力,其余2-3个月恢复。其中3例复发,复发率10.7%,经治疗后恢复。观察组30例患者经护理治疗后均痊愈,其中21例于术后2周即恢复胃肠动力,7例于术后1-2个月恢复,2例术后3个月恢复。经护理治疗后1年内未见复发。2组在术后胃肠动力恢复时间及复发率对比,差异有统计学意义（P〈0.05）。结论食管癌术后胸胃排空障碍以功能性障碍为主,在常规治疗的基础上配合中医针灸调理等显著达到治愈的目的,值得广大基层医疗机构推广应用。Objective To explore the clinical features and etiology of disturbance of intrathoracic gastric emptying after resection of gastric cardial cancer and its nursing measures. Methods The clinical data of 60 patients who underwent functional disturbance of intrathoracic gastric emptying after receiving resection of gastric cardial cancer in our hospital during August 2011 and August2013 was retrospectively analyzed. All of them were divided into control group in which the patients were given conventional nursing, and observation group in which acupuncture and moxibustion therapy were performed based on conventional nursing. The conditions of postoperative emptying and nursing effect of the two groups were compared. Results After nursing intervention, of all the 30 patients in the control group, 28 were cured, 2 underwent reoperation because of exacerbation; 15 got the recovery of gastrointestinal motility within 1 month after operation and the other 15 within 2-3 months; 3 recurred, accounting for 10.7% and were cured after treatment. All the 30 patients in the observation group were cured after nursing with 21 getting the recovery of gastrointestinal motility within 2 weeks after operation, 7 within 1-2 months, 2 within 3 months; no recurrence was found within 1year. There were statistically significant differences between the recovery time gastrointestinal motility and recurrence rate of the two groups（P〈0.05）. Conclusion Acupuncture and moxibustion therapy based on conventional nursing is worthy of promotion widely in basic-level hospitals, by which the functional disturbance of intrathoracic gastric emptying can be effectively treated which is the main type of disturbance of intrathoracic gastric emptying after resection of gastric cardial cancer.
