形状记忆环抱接骨板结合胸腔镜治疗多发肋骨骨折合并血胸治疗效果分析

摘　　要：目的研究应用形状记忆环抱接骨板结合胸腔镜治疗多发肋骨骨折合并血胸的治疗效果。方法整群选取2011年1月—2013年12月2年间在该院进行多发肋骨骨折合并血胸治疗的患者104例,采用随机数表法分为内固定组和非内固定组,各52例,内固定组患者采用内固定治疗。非内固定组患者给予加压包扎和胸部护板外固定。对比两组患者治疗后的胸痛缓解时间、坐起时间、下床时间、住院时间、肺不张、肺部感染、胸壁畸形并发症发生率,治疗前后疼痛评分、PO2含量和PCO2含量。结果内固定组患者在胸痛缓解时间、坐起时间、下床时间、住院时间明显的低于非内固定组,内固定组的肺不张、肺部感染和胸壁畸形并发症发生率明显的低于非内固定组,内固定组患者术后3 d疼痛评分、PO2含量情况均优于非内固定组患者,差异有统计学意义(P<0.05)。结论形状记忆环抱接骨板结合胸腔镜治疗多发肋骨骨折合并血胸具有固定效果好、创伤小、并发症少的优点。Objective To study the efficacy of shape memory encircling bone plate combined with thoracoscopy for multiple rib fractures complicated with hemothorax. Methods 104 patients who received treatment for multiple rib fractures complicated with hemothorax in our hospital during January 2011 and February 2013 were divided into fixation group and non fixation group with52 in each according to random number table. Internal fixation and external fixation by pressure dressing and chest orthosis were performed in the two groups respectively. The chest pain relief time, sitting up time, ambulation time, hospitalization time, the complication rate of pulmonary atelectasis, lung infection, thoracic wall abnormal, and the pain scores, PO2 and PCO2content before and after treatment of the two groups were compared. Results The chest pain relief time, sitting up time, ambulation time, hospitalization time were shorter, the complication rate of pulmonary atelectasis, lung infection, thoracic wall abnormal were lower and the pain scores, and PO2 content 3 days after treatment were better, in the internal fixation than in the non fixation group, and the differences were statistically significant（P〈0.05）. Conclusion Shape memory encircling bone plate combined with thoracoscopy for multiple rib fractures complicated with hemothorax has the advantages of good fixed effects, less trauma and less complication.
