不同麻醉方式与老年下肢骨科手术患者术后肺部感染的分析

摘　　要：目的观察静吸复合全麻和椎管内麻醉下老年下肢骨科手术术后肺部感染的情况。方法整群选取2013年8月—2015年3月间该院共收治需要做下肢骨科手术的高龄患者230例,随机分为观察组和对照组各115例,观察组采用椎管内麻醉、对照组采用静吸复合全麻方式麻醉下行下肢骨科手术,观察两组患者的术后肺部感染情况。结果对照组115例采用静吸复合全麻方式麻醉,术后肺部感染25例,感染率为21.7%,观察组115例采用椎管内麻醉,术后术后肺部感染9例,感染率为7.8%,观察组术后肺部感染率(21.7%)明显低于对照组(7.8%)(P<0.05)。结论采用椎管内麻醉后进行下肢骨科手术能更有效预防肺部感染,术后康复快,适于在高龄患者中使用。Objective To observe the effect of intravenous-inhalation general anesthesia and spinal anesthesia on pulmonary infection in elderly patients undergoing lower-limb orthopedic operation. Methods 230 elderly patients admitted in our hospital during August 2013 and March 2015 who needed lower-limb orthopedic operation were randomly divided into observation group and control group with 115 in each group, respectively given spinal anesthesia and intravenous-inhalation general anesthesia during operation. The situations of postoperative pulmonary infections of the two groups were observed. Results In terms of postoperative pulmonary infections, the number and occurring rate of the control groups were 25 and 21.7%, significant higher than those of the ob-servation group, 9 and 7.8%, and the differences were statistically significant（P〈0.05）. Conclusion Spinal anesthesia is suitable for elderly patients in the lower-limb orthopedic operation because of its effective control of pulmonary infection which can lead to quick recovery.
