早期应用小剂量多巴胺联合呋塞米治疗心功能Ⅲ-Ⅳ级心力衰竭的疗效探究

摘　　要：目的探究早期应用小剂量多巴胺联合呋塞米治疗心功能Ⅲ-Ⅳ级心力衰竭的疗效。方法整群选自2012年8月—2014年12月长春市第二医院心内科就诊的84例心功能Ⅲ-Ⅳ级心力衰竭的患者,随机分为两组,治疗组42例早期给予静脉持续泵入小剂量多巴胺(0.5-1.0ug.kg-1.min-1)联合呋塞米(20-40mg/d)静脉注射治疗,对照组42例仅静脉注射呋塞米(20-40mg/d)。比较两组治疗效果、左室射血分数(LVEF)、左室舒张末内径(LVIDd)和B型利钠肽(BNP)水平。结果治疗组总有效率90.48%(38/42),对照组总有效率73.81%(31/42),两组比较差异有统计学意义(χ2=3.153,P=0.0012)。治疗后两组LVEF、LVIDd、BNP水平均有改善,治疗组改善优于对照组,差异有统计学意义,P<0.05。结论早期应用小剂量多巴胺联合呋塞米可改善心功能Ⅲ-Ⅳ级心力衰竭患者的心功能,提高治疗效果。Objective Curative effect observation on early application in treatment to functional class Ⅲ-Ⅳ heart failure with small dose of dopamine combined with furosemide. Methods 84 patients, with cardiac functional class Ⅲ-Ⅳ heart failure, accepted by the Department of Cardiology, Changchun Second Hospital, from August 2012 to December 2014, were selected and randomly divided into two groups, the treatment group, 42 cases, early made intravenous continuous infusion of small dose dopamine（0.5-1.0 ug·kg-1·min-1） combined with furosemide（20-40mg/d） intravenous injection therapy, while control group, 42 cases,only made intravenous injection of furosemide（20-40mg/d）. Therapeutic effects, left ventricular ejection fraction（LVEF）, left ventricular internal diameter at end-diastole（LVIDd） and B-type natriuretic peptide（BNP） levels were compared. Results The total effective rate of treatment group was 90.48%（38/42）, and the total efficiency of control group 73.81%（31/42）, so the difference was statistically significant（Х^2=3.153, P=0.0012）. LVEF, LVIDd, and BNP levels of the two groups were improved after treatment,and the treatment group＇s improvement was better than that of the control group, with statistical significance, P〈0.05. Conclusion Early application of small dose dopamine combined with furosemide, patients with cardiac functional class Ⅲ-Ⅳ heart failure, cardiac function and therapeutic effect could be improved.
