安络化纤丸治疗原发性胆汁性肝硬化及对患者免疫学指标的影响

摘　　要：目的：探讨安络化纤丸辅助治疗原发性胆汁性肝硬化的疗效及其对患者免疫学指标的影响。方法：选取84例原发性胆汁性肝硬化患者为研究对象,随机分为观察组和对照组,每组42例。对照组：给予甘草酸二铵葡萄糖注射液联合熊去氧胆酸片治疗。观察组：给予安络化纤丸辅助甘草酸二铵葡萄糖注射液联合熊去氧胆酸片治疗。观察比较两组患者疗效、不力引发、临床症状、症状积分、肝功能、肝纤维化指标、门脉血流动力学、Ig G及Ig M水平的变化情况。结果：治疗5、16及24周,观察组黄疸、乏力、皮肤瘙痒及肝区不适等临床症状改善情况均优于对照组,差异均有统计学意义（P〈0.01~0.05）。治疗5、16及24周,观察组症状积分、肝功能ALT、AST、TBIL、肝纤维化相应指标等都低于对照组,差异均有统计学意义（P〈0.01~0.05）。治疗后,观察组门脉血流动力学SV及PV水平变化都优于对照组,差异均有统计学意义（P〈0.05）。治疗24周后,观察组Ig G及Ig M水平都低于对照组,差异均有统计学意义（P〈0.01~0.05）。治疗24周后,观察组显效率71.4%高于对照组显效率45.2%,差异有统计学意义（P〈0.05）。结论：安络化纤丸辅助治疗原发性胆汁性肝硬化疗效显著,能够有效缓解患者黄疸、乏力等临床症状及体征,降低症状积分及肝纤维化指标水平,并可以改善患者的免疫学指标。Objective： To investigate the effect of Anluo Huaxian pill in treatment of primary biliary cirrhosis and intervention on immunological indexes. Method： A total of 84 patients with primary biliary cirrhosis patients as the research object were randomly divided into observation group and control group,42 cases in each group. Control group： Diammonium Glycyrrhizinate and Glucose Injection combined with Ursodeoxycholic Acid Tablets treatment. Observation group： Anluo Huaxian pill with Diammonium Glycyrrhizinate and Glucose Injection combined Ursodeoxycholic Acid Tablets treatment.Observe and compare the situation,effect,clinical symptoms,symptom scores,liver function,hepatic fibrosis,portal blood flow dynamics and the levels of Ig G and Ig M in the two groups. Results： 5,16 and 24 weeks of treatment later,jaundice,fatigue,skin itching,liver discomfort and improvement of clinical symptoms of the observation group were better than those in the control group. The differences were statistically significant（ P〈 0. 01,P〈 0. 05）. After treatment of 5,16 and 24 weeks,symptom score,ALT,AST,TBIL,liver fibrosis indexes of the observation group were lower than the control group＇s. The differences were statistically significant（ P〈 0. 01,P 〈0. 05）. After treatment,portal hemodynamics SV,PV level changes in the observation group were better than those in the control group. The differences were statistically significant（ P 0. 05）. After 24 weeks of treatment,Ig G and Ig M levels in the observation group were lower than the control group＇s and there was statistically significant difference（ P 〈0. 01,P 〈0. 05）. After 24 weeks of treatment,the observation group＇s effective rate was 71. 4% higher than 45. 2% in the control group and the difference was statistically significant（ P 〈0. 05）.
