固本益气滋阴汤对2型糖尿病胰岛β细胞保护功能的临床研究

摘　　要：目的:研究固本益气滋阴汤对2型糖尿病胰岛β细胞保护功能。方法:选取糖尿病气阴两虚证患者300例,随机数字法分为治疗组和对照组,其中治疗组151例;对照组149例。两组患者均在接受糖尿病饮食和常规运动,对照组给予马来酸罗格列酮片口服每天1次,每次1片,治疗组在对照组治疗方案的基础上,给予固本益气滋阴汤每日1剂水煎服,早晚分两次服用。两组均以连续服药8周为1个疗程。比较治疗前后两组症候积分,糖化血红蛋白指数,空腹及餐后2 h血糖和胰岛素水平,以及胰岛β细胞功能评价相关指数胰岛素作用指数(IAI),胰岛素抵抗指数(IR)和HOMA-β指数。结果:两组中医症候积分均比治疗前显著降低(P<0.05),但治疗组中医症候积分下降程度比对照组明显(P<0.05);对照组和治疗组患者空腹和餐后2 h血糖以及糖化血红蛋白指数与治疗前相比均显著降低(P<0.05),对照组和治疗组患者餐后2 h血胰岛素含量与治疗前相比均显著升高(P<0.05),但两组治疗后各时点上述指标相比无显著差异(P>0.05);两组患者治疗后胰岛作用指数IAI和Homa-β指数显著升高(P<0.05),治疗组比对照组升高更显著(P<0.05);两组Homa-IR指数均比治疗前显著下降(P<0.05),治疗组该指数显著低于对照组(P<0.05)。结论:固本益气滋阴汤对2型糖尿病胰岛β细胞有保护功能。Objective： To observe the protecting effect of Gubenyiqiziyin decorction on βcells in pancreas of diabetes patients. Methods： 300 diabetes patients were collected from June 2013 to June 2015. The patients were divided into control group and treatment group randomly. In addition to receive diabetes diet and regular sports exercise,control group received Rosiglitazone maleate tablet 4 mg once every day,while treatment group received the same treatment as control and Gubenyiqiziyin decorction twice a day at the same time. Both group were treated for 8 weeks as a course. Before and after treatment,the TCM symptom scores,Glycated hemoglobin index,fasting plasma glucose（ FPG） and insulin,2-hour postprandial blood glucose and insulin,IAI,Homa-IR,and Homa-βwere tested. Results： After treatment,the TCM symptom scores in both treatment group and control group were significantly decreased（ P〈 0. 05）,while the TCM symptom scores in treatment group were obviously lower than that of control group at the end of the treatment course（ P〈 0. 05）. Glycated hemoglobin index,FPG and 2-hour postprandial glucose were significantly decreased in both group（ P 〈0. 05）. 2-hour postprandial insulin were obviously increased in both group（ P〈 0. 05）,but no different could be found between two groups after treatment（ P 〉0. 05）. IAI and Homa-β in each group were significantly increased after treatment（ P 〈0. 05） and these 2 indexes were much higher in treatment group than that of control group（ P 〈0. 05）. Homa-IR in both group were obviously decreased after treatment and Homa-IR in treatment group was much lower than that of in control after treatment（ P〈 0. 05）. Conclusion： Gubenyiqiziyin decorction has obvious the protecting effect on βcells in pancreas of diabetes patients
