痔上黏膜环形切除术联合外痔切除与外剥内扎术治疗混合痔的效果对比

摘　　要：目的比较吻合器痔上黏膜环形切除术（PPH）联合外痔切除与外剥内扎术（MMH）治疗混合痔的临床效果。方法选取2011年1月-2015年1月在本院治疗的118例混合痔患者作为研究对象,根据手术方式分为观察组和对照组,各59例。观察组采用PPH联合外痔切除术,对照组采用MMH治疗。比较两组的效果。结果观察组的术中出血量显著少于对照组,手术时间、住院时间、创面愈合时间显著短于对照组,住院费用（不含吻合器费用）显著低于对照组,差异有统计学意义（P〈0.05）。观察组的术后并发症评分显著低于对照组,差异有统计学意义（P〈0.05）。结论 PPH联合外痔切除治疗混合痔的临床效果优于MMH,值得临床推广应用。Objective To compare the clinical efficacy of procedure for prolapse and hemorrhoids（PPH） combined with external hemorrhoidectomy and milliganmorgan hemorrhoidectomy（MMH） in the treatment of mixed hemorrhoids.Methods 118 patients with mixed hemorrhoids in our hospital from January 2011 to January 2015 were selected and divided into the observation group and the control group according to the surgical procedures,59 cases in each group.The observation group was treated with PPH combined with external hemorrhoidectomy,the control group was given the conventional MMH.The treatment effect of two groups was compared. Results The amount of bleeding during surgery in the observation group was less than that in the control group,the operative time,hospitalization time,wound healing time in the observation group was shorter than that in the control group,the hospitalization expense（except stapler costs） in the observation group was lower than that in the control group,with significant difference（P〈0.05）.The score of postoperative complication in the observation group was lower than that in the control group,with significant difference（P〈0.05）. Conclusion The effect of PPH combined with external hemorrhoidectomy in the treatment of mixed hemorrhoids is more effective than that the MMH,it is worthy of clinical promotion and application.
