摘  要:目的评估膝骨性关节炎患者关节液中WISP-1的水平,以及与膝关节K-L分级、MRI分级以及软骨outerbridge分级之间的相关性。方法 50例膝关节OA患者及10例急性膝关节损伤患者纳入研究,用ELASI方法测量患者膝关节液中WISP-1的水平,比较两组间WISP-1的表达水平。用K-L分级评价患者膝关节站立位的X线片,用膝关节MRI评价患者的滑膜炎以及软骨损伤程度,通过关节镜观察评价outerbridge的分级及软骨损伤程度。结果 OA患者膝关节液中WISP-1含量为(110.14±50.46)pg/ml高于对照组患者(53.93±24.04)pg/ml,P=0.001。OA患者膝关节液中WISP-1的含量与全膝关节滑膜炎MRI分级存在相关性(r=0.767,P<0.001),与软骨损伤无相关性。结论 OA患者膝关节液中WISP-1的表达水平与滑膜炎的严重程度相关,WISP-1可能是OA药物治疗的靶点。Objective To investigate the synovial levels of WISP-1 in patients with knee osteoarthritis,and their relationship with Kellgren/Lawrence score,MRI grade and outerbridge grade. Methods Fifty patients with knee OA and 10 patients with acute knee joint trauma controls were enrolled in this study. WISP-1 levels in synovial fluid were measured using enzyme-linked immunosorbent assay. The radiographic grading of OA in the knee was performed by using the Kellgren-Lawrence criteria,synovitis severity and the loss of cartilage were assessed using mri,the damage of cartilage was assessed by arthroscopy and arthrotomy,and classed by outerbridge classification. Results Compared to the controls,OA patients had higher WISP-1 concentration in synovial fluid[(110.14±50.46)pg/ml vs(53.93±24.04)pg/ml],P=0.001. The synovial fluid WISP-1 concentrations were not correlated with radiographic severity and the the loss of cartilage. And synovial fluid WISP-1 levels significantly correlated with severity of synovitis(r=0.767,P 0.001). Conclusion WISP-1 in synovial fluid is associated with severity of synovitis and is likely to be a may be a novel target for drug intervention in knee OA.
