摘　　要：目的探究1,3-β-D-葡聚糖(G试验)与痰培养对临床肺部曲霉菌感染的诊断价值,为临床治疗提供参考依据。方法回顾性分析2013年1月1日-12月31日呼吸系统感染患者送检27 560份痰液标本,150份曲霉菌属检出阳性,其中有41份进行G试验,比较G试验、痰培养与CT诊断结果之间的一致性,数据采用SPSS17.0软件进行统计分析。结果 41份曲霉菌属阳性标本中G试验结果阳性12份符合率29.3%,CT结果阳性28份符合率68.3%;G试验结果与痰培养及CT结果均一致有9份,CT与G试验对肺曲霉菌的诊断差异有统计学意义(P<0.005),CT阳性率高于G试验。结论 G试验与痰培养及CT检查之间的符合率低,不能作为肺曲霉菌感染的诊断指标,痰培养仍不失为肺曲霉菌感染的重要参考方法。OBJECTIVE To study the actual efficacy of （1,3）-β-D-glucan test （serum BG assay ） and sputum cul- ture in diagnosing pulmonary aspergillosis so as to provide references for clinical treatments. METHODS Totally 150 Aspergillus positive cases from 27 560 sputa culture isolated from patients with respiratory tract infections from Jan. 2013 to Dec. 2013 were retrospectively analyzed. Among them, 41 cases received serum G assay. The consistency of the results of G assay, sputum culture and CT were compared. All data were statistically analyzed by SPSS 17.0 software. RESULTS Among the 41 positive cases, 12 cases were found to be positive in G assay and the accordance rate was 29.3%. Totally 28 cases were found to be positive by CT diagnosis and the accordance rate was 68.3%. The results of 9 cases were found to be consistent through the three methods and the diagnostic differences of CT and G assay were significant （P〈0. 005）. The positive rate by CT was higher than that by G as- say. CONCLUSION The coincidence rate of the results tested by serum G assaY, CT, and sputum culture was too low so that serum G assay counld not be chosen as a diagnostic index for pulmonary aspergillosis and sputum cul- ture was still an important method for diagnosing pulmonary aspergillosis.
