ICU感染患者降钙素原变化的临床价值研究

摘　　要：目的探讨ICU感染患者降钙素原变化的临床价值分析,为评价患者感染程度提供依据。方法选择医院2014年2月-2015年5月入住ICU患者80例进行研究,其中发生医院感染的患者42例为观察组,未发生医院感染的患者38例为对照组,两组患者均进行降钙素原水平检测,对比其结果,采用SPSS 18.0软件进行统计分析。结果观察组患者第1、2、4、6天的降钙素原水平明显高于对照组,差异有统计学意义;当阈值为0.5μg/L时测量降钙素原水平,其敏感度94.73%、特异度90.24%、阳性预测值90.47%、阴性预测值97.36%;当阈值为2.0μg/L时测量降钙素原水平,其敏感度95.23%、特异度94.92%、阳性预测值90.47%、阴性预测值94.73%;观察组患者降钙素原水平检测的阳性率为95.23%,显著高于对照组患者阳性率5.26%,两组差异有统计学意义(P<0.05)。结论降钙素原的变化能对ICU感染患者的感染程度进行全面的反映,对于ICU患者是否发生感染的判断具有重要的临床价值,值得在临床上推广应用。OBJECTIVE To explore the clinical value of change of procalcitonin in the patients with infections of ICU so as to provide guidance for evaluation of the degree of infections. METHODS A total of 80 patients who were hos- pitalized the ICU from Feb 2014 to May 2015 were enrolled in the study; 42 patients with nosocomial infections were assigned as the observation group, and 38 patients without nosocomial infection were set as the control group. The procalcitonin levels were detected and compared between the two groups of patients, and the statistical analysis was performed with the use of SPSS18.0 software. RESULTS The procalcitonin levels of the observation group were significantly higher than those of the control group on Day 1, 2, 4, and 6, When the threshold value was 0 . 59 g/L, the sensitivity of the procalcitonin level was 94.73%, the specificity 90.24%, the positive pre- dictive value 90.47%, the negative predictive value 97.36%. When the threshold value was 2. 0 μg/L, the sen- sitivity of the procalcitonin level was 95.23 %, the specificity 94.92 %, the positive predictive value 90.47 %, the negative predictive value 94. 73 %. The positive rate of the detection with procalcitonin level of the observation group was 95.23%, significantly higher than 5.26% of the control group （P〈0.05）. CONCLUSION The change of procalcitonin can fully reflect the degree of infections in the ICU patients, and it has significant clinical value in judgment of infections in the ICU patients and is worthy to be promoted in the hospital.
