摘 要:目的探讨持续负压吸引术和湿性换药治疗糖尿病足并发严重感染患者的临床疗效,以改善愈后。方法选取2009年1月-2015年1月收治的88例糖尿病足并发严重感染患者为研究对象,根据患者采用的治疗方法将其分为观察组和对照组,每组各44例;观察组采用持续负压吸引治疗,对照组采用湿性换药治疗,对比两组患者治疗后的疗效及临床资料,数据采用SPSS 19.0软件进行统计分析。结果观察组和对照组患者的临床总有效率分别为100.00%和72.73%;治疗后观察组患者的空腹血糖、餐后2h血糖和HbA1c均显著低于对照组,观察组患者的保肢率显著高于对照组患者,观察组患者的截肢率和病死率均低于对照组患者,比较两组间差异有统计学意义(P<0.05)。结论给予糖尿病足并发严重感染患者持续负压吸引治疗的临床疗效优于湿性换药治疗,能较好的控制患者的血压,降低截肢率和病死率。OBJECTIVE To explore the clinical effect of continuous vacuum aspiration and wet dressing in the treat- ment of diabetic foot patients with severe infections so as to improve prognosis. METHODS A total of 88 cases of severe infected patients with diabetic foot were chosen from Jan. 2009 to Jan. 2015 as study objects and they were divided into observation group and control group according to the different treatments with 44 cases each. The ob- servation group was treated with continuous suction method and the control group adopted wet dressing treatment. The curative efficacy and clinical data of the two groups were compared. SPSS 19.0 was adopted for statistical a- nalysis. RESULTS Total clinical effective rates in the observation group and the control group of patients were 72.73 % and 100.00%, respectively. After the treatments, fasting blood glucose, postprandial 2 h plasma glucose and HbAlc levels in the observation group were significantly lower than the control group, and patients in the ob- servation group had a significantly higher limb salvage rate. The amputation and mortality rates of the observation group were lower than the control group and the differences between the two groups were significant (P〈0.05). CONCLUSION The clinical efficacy of continuous suction therapy is better than wet dressing treatment in severe infected patients with diabetic foot for it can control blood pressure and reduce the rate of amputation and mortali- ty.
