摘　　要：目的研究ICU导管相关性感染的目标性监测及与医院感染相关性,为ICU患者诊治提供参考依据。方法回顾性分析2012年1-12月入住ICU且留置导管患者共131例,纳入对照组,另选取医院2013年1-12月入住ICU且留置导管患者共131例,纳入观察组;实施目标性监测后,对比患者导管感染率、病原菌分布等,数据采用SPSS 13.0软件进行统计分析。结果观察组患者医院感染率为8.4%,显著低于对照组的16.8%,差异有统计学意义(P<0.05);2013年共检出32株病原菌、2012年共检出43株,均以鲍氏不动杆菌为主,分别占15.6%、25.6%,2013年整体比2012年病原菌检出低(P<0.05);根据Spearman法分析可知,随着ICU留置导管时间的增长,感染率亦逐渐增加,二者呈正相关。结论对ICU导管感染进行目标性监测,可以有效降低医院感染的发生,值得在临床上推广应用。OBJECTIVE To study the objective monitoring of catheter related infections in ICU and the correlation of monitoring and hospital infections so as to provide reference for treatment of ICU patients. METHODS A retro- spective analysis was conducted. A total of 131 patients admitted in ICU who received indwelling catheter during Jan. to Dec. 2012 were enrolled in the control group and another 131 such patients during Jan. to Dec. 2013 were enrolled in the observation group. Objective monitoring was performed and the two groups were compared for catheter infection rate, pathogen distribution and so on. The software SPSS 13.0 was used for statistical analysis. RESULTS The hospital infection rate was 8.4% in the observation group, significantly lower than 16.8% in the control group （P〈0.05）. Totally 32 pathogens and 43 pathogens were detected respectively in the year 2013 and 2012. Acinetobacter baurnannii was the prevalent pathogens in both years, accounting for 15.6% and 25, 6 % in each year. Overall, the amount of pathogens in 2013 was lower than in 2012 （P〈0. 05）. According to the analy- sis by Spearman method, along with the extension of the ICU indwelling time, the infection rate increased gradual- ly in a positive correlation. CONCLUSION Objective monitoring of catheter infections in the ICU can effectively re- duce the occurrence of hospital infections, worthy of popularization and application in clinic.
