摘 要:目的分析胫骨平台骨折患者术后切口感染的临床特点,为临床治疗提供参考依据。方法选择从2012年3月-2014年1月于医院接受治疗的50例胫骨平台骨折患者作为研究病例,观察术后切口感染及病原菌分布情况;并分析影响因素其感染的相关因素,数据采用SPSS 18.0软件进行统计分析。结果 50例患者中共13例患者发生切口感染,感染率为26.0%;13例感染患者共培养出13株病原菌,以革兰阳性菌为主,共10株占76.9%;患者术后两周内发生切口感染率显著高于术后>2周的患者,差异有统计学意义(P<0.05);手术时机、术后处理、术后引流、骨折类型、抗菌药物等为患者发生切口感染的相关危险因素(P<0.05)。结论胫骨平台骨折患者术后切口感染的危险因素较多,围术期应对危险因素给予合适的护理措施,可以显著降低术后切口感染率。OBJECTIVE To analyze the clinical characteristics of the tibial plateau fractures patients with postopera- tive incision infections so as to provide guidance for clinical treatment. METHODS A total of 50 patients with tibial plateau fractures who were treated in the hospital from Mar 2012 to Jan 2014 were enrolled in the study, then the incidence of postoperative incision infections and distribution of pathogens were observed, the influencing factors for the infections were analyzed, and the statistical analysis of data was performed with the use of SPSS 18.0 soft- ware. RESULTS The incision infections occurred in 13 of 50 patients, with the infection rate of 26.0%. Totally 13 strains of pathogens were isolated from the patients with the infections, of which (76. 9%) were gram-positive bacteria. The incidence of incision infections was significantly higher after the surgery for less than 2 weeks than after the surgery for more than 2 weeks (P〈0.05). The related risk factors for the incision infections included the timing of surgery, postoperative management, postoperative drainage, types of fracture, and antibiotics (P〈 0.05). CONCLUSION There are a variety of risk factors for the postoperative incision infections in the patients with tihial plateau fractures. It is necessary to take the appropriate nursing measures during the perioperative peri- od so as to significantly reduce the incidence of the postoperative incision infections.
