摘　　要：目的研究P16INK4A蛋白在云南省HIV感染妇女宫颈上皮内瘤病变的表达,而探讨其对高度宫颈上皮内瘤病变的预测价值.方法调取67例已确诊为艾滋病病毒感染妇女的宫颈组织活检的病理切片作为研究对象,其中13例的病检结果提示无宫颈上皮内瘤病变的宫颈组织作为对照组.检测各级别宫颈上皮内瘤组中P16INK4A蛋白的表达情况,并分析P16INK4A蛋白对宫颈病变的预测价值.结果低级别宫颈上皮内瘤变(CIN1)组与高级别宫颈上皮内瘤变(≥CIN2)组P16INK4A蛋白的表达率差异有统计学意义(χ2=20.053;P<0.05),说明P16INK4A蛋白能辨别高级别的宫颈上皮内瘤变;随着宫颈上皮内瘤变的级别升高,P16INK4A蛋白的表达强度逐渐增强,呈线性相关,相关系数R=0.397(P<0.05).结论 pl6l NK4A蛋白免疫组化检测对宫颈上皮内瘤变(高级别)有预测价值,能将高级别的宫颈上皮内瘤变区分出来,对HIV感染妇女宫颈疾病的进展有预测价值,同时具有较高的临床治疗指导作用.Objective To study PI6INK4A protein expression in intraepithelial neoplasia lesions and investigate its predictive value in the high-grade cervical intraepithelial neoplasia lesions among HIV-infected women in Yunnan Province, China. Methods 67 biopsies of cervical tissue from HIV-infection women were taken as study objects, including 13 cases of non-cervical intraepithelial neoplasia lesions as the control group. The P16INK4A protein expression in cervical intraepithelial neoplasia was detected and the predictive value of P16INK4A protein in cervical lesions was analyzed. Results The P16INK4A protein expression showed statistically significant difference between low-grade cervical intraepithelial neoplasia （CIN 1） group and high-level cervical intraepithelial neoplasia （≥CIN2） group （ X2 = 20.053, P 〈 0.05） , which suggested P16INK4A protein could identify high-level cervical intraepithelial epithelium. With the increasing grade of cervical intraepithelial neoplasia, the protein expression intensity of P16INK4A was gradually increased with linear correlation coefficient R = 0.397 （P 〈0.05） Conclusion P161NK4A immunohistochemical detection has predictive value in cervical intraepithelial neoplasia （high-grade） and cervical disease progress in HIV-infected women. It can distinguish the high-grade cervical intraepithelial neoplasia and provide guidance for clinical treatment.
