胎儿永久性右脐静脉超声诊断及预后评估

摘　　要：目的:探讨胎儿永久性右脐静脉(PRUV)的超声特征及预后情况。方法:超声筛查发现胎儿PRUV 84例,分析超声特征,合并其他畸形或脐带、羊水异常情况。并跟踪随访至胎儿出生。结果:超声筛查38 015例中发现胎儿PRUV 84例,检出率为0.22%。其中孤立性胎儿PRUV 77例,成功随访69例,出生后发育良好;合并畸形或脐带、羊水等异常7例,2例仅脐带、羊水异常者预后良好,1例左侧多囊性发育不良肾择期手术,余4例多发严重畸形胎儿引产。结论:胎儿PRUV有其特有的声像表现,超声是有效的诊断方法。孤立性PRUV较多,且预后良好;合并畸形者常为多发畸形、严重畸形,预后不佳。Objective： To explore the sonographic features and prognosis of fetal permanent right umbilical vein（ PRUV）. Methods：Eighty-four cases of fetal PRUV diagnosed by ultrasound were collected,the ultrasonic features and other deformity or umbilical cord,amniotic fluid anomalies were analyzed with relevant follow-up. Results： Eighty-four cases of fetal PRUV were found by ultrasound in38 015 pregnant cases,the detection rate was 0. 22%. In these cases,77 fetuses were isolated PRUV,69 cases of them were followed up and had favorable prognosis; 7 fetuses combined with malformation or other abnormalies were detected by ultrasound,among them,2 fetuses who had umbilical cord,amniotic fluid anomalies had the favorable prognosis,1 fetuse with left multicystic dysplastic kidney was underwent elective surgery,the remaining 4 cases of multiple serious malformation fetus were underwent induced labour.Conclusions： Fetal permanent right umbilical vein has unique ultrasonic characteristic,ultrasound is an effective diagnostic method,isolated PRUV accounted for the vast majority,and has the favorable prognosis,a few combined with multiple deformities and severe deformities always has the poor prognosis.
