南京城区居民社区护理需求现状及影响因素

摘　　要：目的分析南京城区居民社区护理需求状况及其影响因素,为发展社区护理工作提供依据。方法随机选择南京市5个社区常住居民为调查对象,采用入户调查方式,对居民人口学资料、居民寻求的健康行为方式、疾病预防和健康促进护理需求进行调查。结果共调查社区居民519户,疾病护理需求率最高的是院前急救护理(86.1%);疾病预防护理需求率最高的是儿童预防接种与建立系统管理(76.7%);健康促进护理需求率最高的是急救知识宣教(80.9%)。与疾病护理需求呈正相关的是家庭成员有无慢性病、家中有无老人及人均收入;家庭成员医疗保险状况及住所与医疗机构的距离与居民疾病护理需求呈负相关。结论慢病人群、老年人群、人均收入高的家庭有一定的疾病护理需求,应进一步推广全民医疗保险,合理配置卫生资源,提高社区护理人员综合服务能力,改善社区护理服务的可及性。Objective To analyze current situation of community nursing care service demands and its influence factors; to provide basis to promote community nursing care service. Methods Permanent residents in 5 communities in Nanjing were selected randomly as survey subjects, household interviews were conducted to investigate demographic information, residents＇ health-seeking behaviors and nursing care service demands for disease prevention and health promotion. Results A total of 519 families were investigated. The highest demand rate of disease nursing care , disease prevention care and health promotion nursing care was pre-hospital emergency care （86.1% ）, children＇s vaccination and system management establishment （76.7 % ） and publicity on first-aid knowledge （80.9%）, respectively. Chronic disease status of family members, with or without the aged family members and average income were positively correlated with demands for disease care service; while family members＂ medical insurance status and distance to medical facilities were negatively correlated with demands for disease nursing care service. Conclusion Residents with chronic diseases, aged persons and families with high average income had certain demands for disease nursing care service. Promotion of medical insurance coverage and reasonable allocation of health resources, together with improvement of comprehensive service ability of community nursing staff would increase the aeessibility of community nursing care service.
