摘　　要：目的:通过基于社区的糖尿病监护系统对糖尿病生活方式的干预,改变糖尿病患者的不良生活方式,延缓糖尿病并发症的发生和发展,提高患者的生活质量。方法:建立患者电子档案,采用互联网和移动医疗技术构建医院指导、社区干预、患者自我管理相结合的个体化慢病照护体系,通过智能手机实现对患者饮食、运动、用药进行监测和提醒,并对患者进行糖尿病基础知识、预防并发症方面进行指导教育。结果:通过基于社区的糖尿病监护系统监测和干预,患者的用药习惯和生活方式有显著改变。结论:监护系统从用药、生活习惯、健康教育等多方面对患者干预和指导,有效地控制了糖尿病并发症的发生和发展,提高了患者生活质量。Objective To develop a community-based diabetes monitoring system to delay the occurrence and development of the complications. Methods Patient electronic records were established, and technologies of Internet and mobile medicine were used to construct an individualized chronic disease care system for hospital instruction, community intervention and patient self-management, which could realize monitoring and reminding of diet, exercise and medication by the smart phone and guide the patient in diabetes knowledge and complications prevention. Results The system improved the medication and life style of the patient. Conclusion The system contributes to the prevention and mitigation of the diabetes complications, and thus enhances the living quality of the patient.
