摘  要:目的:研究高海拔地区新兵体能训练与膝关节损伤的相关性,为减少其发生提供理论依据。方法:收集不同兵源地的180名新兵训练第1-3月(每一个月为一阶段)的右膝关节MRI资料,观察各阶段的膝关节骨质(骨髓、骨皮质、关节软骨)、半月板、韧带、软组织及关节腔损伤情况,并进行不同入伍地、入伍前参加体能活动情况及体质量分级间比较。结果:(1)低海拔地区新兵第一阶段骨髓水肿及关节腔积液检出率高于高海拔地区(P〈0.05)。(2)入伍前是否参加体育锻炼及体质量分级对各训练阶段膝关节骨质、半月板、韧带、软组织及关节腔损伤影响差异均无统计学意义(P〉0.05)。(3)不同阶段的骨髓损伤检出率差别有统计学意义(P〈0.05),第三阶段的检出率最高;第一阶段和第二、三阶段半月板损伤检出率差别有统计学意义(P〈0.05),第三阶段的检出率最高;第一阶段和第二、三阶段关节腔损伤检出率差别有统计学意义(P〈0.05),第三阶段的检出率最高;各阶段膝关节韧带及软组织损伤差异均无统计学意义(P〉0.05)。结论:MRI检查可为制订正确防护措施和科学的施训策略提供重要依据,从而最大限度地减少和避免新兵训练损伤的发生和发展。Objective To study the correlation between the physical fitness training and knee joint injury of the recruit at high altitude so as to decrease the incidence rate of the injury. Methods Totally 180 recruits with different sources underwent MRI examinations monthly at the first three months of fundamental training. The lesions at all months were observed for the parts of knee joints including bone marrow, cortical bone, articular cartilage, meniscus, ligaments, soft tissues and articular cavity, and compared from the aspects of source, physical fitness before recruiting and body mass grading. Results The recruits from low-altitude area gained significantly higher detection rate of bone marrow edema than those from high-altitude area at the first month, with P〈0.05. Physical fitness before recruiting and body mass grading had no significant influences on the lesions of knee joint sclerotin, meniscus, ligaments, soft tissues and articular cavity,with P〉 0.05. There were statistical differences between the detection rates of bone marrow lesions at different months,with P〈0.05, and the detection rate gained the peak value at the third month. The detection rates of meniscus lesion and articular cavity lesion at the first month were significantly different from those at the second and third month, with P〈0.05, and the detection rates gained the peak values at the third month. There were no statistical differences between the detection rates of the lesions of ligaments and soft tissues at all the months, with P〉0.05. Conclusion MRI may contribute to developing proper training strategies for protection and training to decrease or avoid training injury.
