摘 要:目的探讨孕期大鼠急性胰腺炎病因及病理变化的相关情况。方法选择2013年2月至2015年1月由湖北医药学院附属东风医院实验动物中心提供的健康性成熟SD雌性大鼠48只,雄性大鼠24只,按雌雄比2∶1合笼进行交配,发现有阴道栓后记为孕期第1日,根据随机数字表法分为实验组和对照组,各24只。对照组孕后继续进行自由喂养,而实验组在孕期第7日建立急性胰腺炎模型。实验组在造模后6、12、24 h(对照组在相应时间点)进行肺湿重系数测定、肺组织的组织学评分、肺组织细胞凋亡测定及肺组织Bax和Bcl-2 mRNA测定,并进行相关性分析。结果病理学检查发现,与对照组相比实验组的胰腺组织坏死明显,大量炎性细胞浸润,造模获得成功。实验组造模后相应时间点的肺湿重系数、肺损伤病理评分较对照组均显著升高(P<0.01)。对照组肺组织仅见少量细胞凋亡,实验组肺组织细胞凋亡指数较对照组显著增加(P<0.01)。对照组的Bax和Bcl-2mRNA表达均较弱,而实验组的Bax和Bcl-2 mRNA表达较对照组各时点均显著增强(P<0.01)。在实验组中,Pearson相关分析显示肺损伤病理学评分、肺湿重系数与肺组织细胞凋亡指数均呈正相关(P<0.05)。结论孕期大鼠急性胰腺炎可导致肺损伤的发生、发展,而Bcl-2、Bax基因表达的改变参与了孕期大鼠急性胰腺炎肺损伤,可能是促进肺组织细胞凋亡的重要因素。Objective To investigate the etiology and pathological changes of acute pancreatitis in pregnancy rats. Methods From Feb. 2013 to Jan. 2015 from Dongfeng General Hospital Experimental Animal Center,48 sexually mature female SD rats and 24 male rats, according to male and female ratio 2:1 were put into cage mate, recoding as day 1 of pregnancy on the discovery of vaginal suppository, and were equally divided into experimental group and control group of 24 rats according to random number table method. The control group after pregnancy received continuous free feeding, the experimental group was set up acute pan- creatitis model on gestation day 7. After modeling of 6,12,24 h in the experimental group (control group at the corresponding time points), the lung wet weight index, iung histological scores, apoptosis of lung tissue, the bax and Bcl-2 mRNA levels were measured,and correlation analysis was conducted. Results Compared with the control group, pathologic examination revealed evident pancreatic tissue necrosis in the experimental group, inflammatory cell infiltration, suggesting successful modeling. Experimental lung wet weight coefficients and pathologic lung injury score at the time points were significantly increased than the control group ( P 〈 0.01 ). In the control group only a small amount of lung tissue apoptosis was seen, the experimental group was significantly higher than the control group( P 〈0.01 ). Bax and Bcl-2 mRNA expression of the control group was weak, while the experimental group was significantly increased than the control group at each time point ( P 〈 0. 01 ). In the experimental group, Pearson correlation analysis showed that pathological score of lung injury, lung wet weight index and lung apoptosis index were positively correlated(P 〈 0. 05 ).
