基于癌毒病机理论的癌痛平胶囊治疗癌性疼痛的研究

摘　　要：癌毒病机理论是本课题组在继承创新国医大师周仲瑛教授提出的"癌毒"学说的基础上创建的中医肿瘤病机理论体系,癌痛平胶囊是在癌毒病机理论指导下,针对癌性疼痛"癌毒内郁、痰瘀互结、经络雍塞"的核心病机病证创制的治疗癌性疼痛的中药复方制剂。临床研究表明,癌痛平胶囊能够减轻癌痛的疼痛强度、减少疼痛次数、缩短疼痛持续时间,延长止痛作用维持时间,并能改善生活质量、降低不良反应的发生;实验研究证实癌痛平胶囊具有镇痛、减轻瘤重、增强免疫功能等作用,其作用机制可能与降低下丘脑、垂体、腰髓NO含量,提高外周血中β-EP及降低血清5-HT、PGE2的含量、抑制COX活性、抑制脊髓TNF-α、IL-1表达,减少c-fos、SP的释放等有关。临床与实验研究结果显示,癌痛平胶囊治疗癌性疼痛临床疗效确切、作用机制明确、使用安全,具有较好的产业化前景,开发应用将取得显著的经济与社会效益。Cancerous toxin pathogenesis theory was a new theory system of Chinese medicine tumor pathogenesis based on cancerous toxin academic thoughts proposed by Chinese medicine master professor ZHOU Zhong-ying. Under a guidance of the cancerous toxin pathogenesis theory, Aitongping Capsule, a compound Chinese medicinal pharmaceutics for treating cancerous pain, was invented to point at the key pathogenesis that ‘cancer toxin stagnation, coexistence of phlegm and blood stasis, and congestion of channels＇. Clinical studies indicated Aitongping Capsule could reduce the intensity and frequency of cancerous pain, shorten the duration of pain, prolong analgesic effect and reduce adverse reactions to improve the quality of life. Experimental studies indicated that Aitongping Capsule had effects on analgesia, reducing tumor weight and enhancing immune function. The mechanism might be related to down-regulation of some factors, such as NO in hypothalamus, hypophysis and lumbar cord, 5-HT and prostaglandin E2 in the peripheral blood, the expressions of COX and c-fos, the expressions of tumor necrosis factor alpha and interleukin 1 in spinal cord, and substance P, and up-regulation of some factors, such as β-EP in the peripheral blood. The clinical and experimental studies showed Aitongping Capsule had a dei nite curative effect, clear mechanism and assured security. Development and application of Aitongping Capsule could achieve remarkable economic and social benei ts, consequently Aitongping Capsule had a fine industrialization prospect.
