摘　　要：目的:观察调肝理脾方对非酒精性脂肪肝(NAFLD)肝郁脾虚证患者脂联素、抵抗素及肿瘤坏死因子α的影响。方法:将96例肝郁脾虚证NAFLD患者分为治疗组和对照组,其中治疗组64例,对照组32例,分别采用调肝理脾方及安慰剂治疗,疗程均为12周。观察比较两组患者治疗前后脂联素、抵抗素和肿瘤坏死因子α的变化。结果:1治疗前两组脂联素比较无显著性差异;治疗组治疗后脂联素较治疗前明显升高(P<0.05),对照组治疗前后比较无显著性差异;治疗后两组脂联素水平比较有显著性差异(P<0.05)。2治疗组治疗后抵抗素较治疗前明显降低(P<0.05);对照组治疗前后比较,无显著性差异。3治疗组治疗后肿瘤坏死因子α较治疗前明显降低(P<0.05);对照组治疗前后无明显变化;治疗后两组比较有显著性差异(P<0.05)。结论:调肝理脾方配合生活方式干预治疗NAFLD肝郁脾虚证取得良好的临床疗效,这可能是通过升高脂联素水平、降低抵抗素及肿瘤坏死因子α水平实现的。Objective： To observe the effects of Tiaogan Lipi Formula on adiponectin, resistin and TNF-α in nonalcoholic fatty liver disease（NAFLD） of liver stagnation and spleen deficiency. Methods： Ninety-six patients with NAFLD of liver stagnation and spleen deficiency were randomly assigned to treatment group（64 patients） and control group（32 patients）. Two groups were respectively Treated with Tiaogan Lipi Formula and placebo and both the course of treatment were 12 weeks. Adiponectin, resistin and TNF-α before and after the treatment in the two groups were observed. Results： ①Before treatment, the levels of adiponectin in two groups had no statistical differences. After treatment, adiponectin in treatment group was higher than that in control group（P〈0.05）, and higher than that before treatment（P〈0.05）. There was no statistical difference in control group before and after treatment. ②Resistin in treatment group after treatment was lower than that before treatment（P〈0.05）; There was no statistical difference in control group before and after treatment. ③TNF-α in treatment group after treatment was lower than that before treatment（P〈0.05）; There was no statistical difference in control group before and after treatment. After treatment, changes in the two groups had statistical difference（P〈0.05）. Conclusion： Treatment with tiaoganlipi formula plus lifestyle intervention on NAFLD achieved a good clinical curative effect. This maybe through elevating adiponectin levels, lowering the levels of resistin and TNF-α.
