唇裂术后鼻畸形患者的综合鼻整形

摘　　要：目的探讨自体肋软骨联合耳甲腔软骨移植在唇裂术后鼻畸形患者综合鼻整形中的应用。方法以2014年1月至2015年6月在我科行唇裂术后唇畸形修复同期综合鼻整形的6例患者为研究对象，其中女性4例，男性2例。手术选择开放式鼻整形入路，根据不同的鼻畸形程度解剖复位畸形的鼻翼软骨、鼻中隔软骨及肌肉异常附丽，微创切取自体肋软骨联合耳甲腔软骨制作支架复合体，同期行综合鼻整形。结果6例患者术后均得到明显改善，随访1～15个月，唇畸形的修复及综合鼻整形均取得满意效果。结论应用自体肋软骨联合耳甲腔软骨移植，在唇裂术后鼻畸形患者同期综合鼻整形中可以取得满意的效果。Objective To discuss the application of autogenous rib and auricular concha cartilage grafting in rhinoplasty of nasal deformity after cleft lip operation. Methods From Jan. 2014 to June 2015, 6 cases （male 2, female 4） with nasal deformity after cleft lip operation were performed the comprehensive rhinoplasty by autogenous rib and auricular concha cartilage grafting. Under open rhinoplasty approach, the deformed nasal alar cartilage, nasal septal cartilage and abnormally attached muscles were replaced anatomically, the autogenous rib and auricular concha cartilage were harvested to foem the support complex and then the rhinoplasty was carried out simultaneously. Results Following up was performed for 1 to 15 months, all were satisfied with the anatomically corrected nasal deformity and the comprehensive rhinoplasty. Concluion Application of autogenous rib and auricular concha cartilage grafting can receive satisfactory result in rhinoplasty of the nasal deformity after deft lip operation.
