HATCH 评分对心房颤动射频消融术后疗效的预测价值

摘　　要：【目的】探讨H AT C H 评分对心房颤动消融术后疗效的预测价值。【方法】选取2010年12月至2013年12月间在本院接受三维电磁导管定位系统（CART O ）标测下房颤射频消融术的118例患者，随访1年后根据是否维持窦率将患者分为复发组和未复发组。对所有患者均进行 HATCH 评分，并对一般资料、心脏彩超等结果进行相关统计学分析。【结果】左心房内径（LAD）、HATCH 评分、高敏C反应蛋白（hs‐CRP）水平、高血压及脑卒中患者比例组间比较，差异均有统计学意义（ P ＜00．5，P ＜00．1）。多因素回归分析结果显示，HATCH评分和LAD为房颤消融术后复发的独立影响因子。不同 HATCH 评分患者的房颤复发率曲线显示 HATCH≥2分者累积复发率明显高于 HATCH 评分0分和1分者（ P ＜00．1）。【结论】 HATCH ≥2分者行房颤射频消融术后复发的可能性较高，HATCH 评分对于评价房颤消融术后的疗效有一定的预测价值。Objective]To evaluate the importance of the HATCH score on predicting effects of atrial fi‐brillation radiofrequency catheter ablation （AFRCA ） .[Methods] A retrospective analysis was carried out for 118 cases who underwent AFRCA with CARTO guidance from Dec .2010 to Dec .2013 .They were divided into a recurrence group and a non‐recurrence group based on one year follow‐up of sinus rhythm preservation .The clinical data and HATCH scores of the two groups were analyzed .[Results] The ratio of patients with hyper‐tension ,diabetes and stroke were higher in the recurrence group ,and hs‐CRP ,LAD and HATCH scores were significantly higher in the recurrence group compared to those in the non‐recurrence group（ P 〈0 0.5 and P 〈0 0.1） .Furthermore ,Multivariate Logistic regression analysis presented that increased HATCH score and LAD were independent influence factors of atrial fibrillation （AF） recurrence .The AF recurrence curves show that the cumulative recurrence rate of patients with a HATCH score ≥2 was significantly higher than that of the non‐recurrence patients with a HATCH score of 0 or 1（ P 〈0 0.1） .[Conclusion]When the HATCH score of the AF patients≥2 ,the recurrence risk after AFRCA is significantly elevated .So ,HATCH score may be an effective predictor of the success rate of AFRCA .
