高频超声在青年女性乳腺良恶性钙化结节中的诊断价值

摘　　要：【目的】探讨高频彩色超声多普勒在青年女性良恶性乳腺结节中的诊断价值。【方法】回顾性分析103例青年女性乳腺钙化结节的术前超声声像图特征和病理结果，根据病理结果分为良性病变组和恶性病变组，比较两组乳腺钙化结节钙化类型、后方回声有无衰减、血流参数等超声特征，并与病理结果对照。【结果】103例患者术后病理证实：乳腺良性疾病82例，乳腺恶性病变21例；乳腺恶性病变组微钙化、肿块后方回声衰减、肿块血流阻力指数（RI）≥07．的发生率明显高于乳腺良性病变组，差异有统计学意义（ P ＜00．5）。以微钙化为超声特征判断青年女性钙化结节良恶性的符合率为883．％（91／103），误诊率36．6（3／82）％，漏诊率428．％（9／21）；以后方声衰减为超声特征判断青年女性钙化结节性质的符合率为893．％（92／103），误诊率73．2％（6／82），漏诊率238．％（5／21）；以RI≥07．为超声特征判断青年女性钙化结节良恶性的符合率为942．％（97／103），误诊率24．4％（2／82），漏诊率190．％（4／21）（ P ＜00．5）。【结论】青年乳腺钙化结节合并微钙化、后方回声衰减及RI≥07．均提示结节与乳腺恶性病变可能有较高的相关性。Objective]To investigate the diagnostic value of Ultrosonography for the breast calcification nod‐ules in young women .[Methods]A retrospective analysis of preoperative ultrasonographic features and pathologi‐cal results in 103 cases of young women＆#39;s breast calcification nodules was taken and the coincidence rate between ultrasonographic image characteristic and pathology diagnosis was calculate [.Results]In the 103 cases ,according to pathological diagnosis ,there were a total of 82 cases of benign breast diseases and 21 cases of malignant breast diseases .The incidence of micro‐calcifications ,echo attenuation ,and resistance index （RI） ≥0 7. in the malignant breast tumors was significantly higher than that in the benign breast diseases （ P ＆lt;0 0.5） .Using the incidence of micro‐calcifications ,correct diagnosis was 88 4.% （91/103） ,misdiagnosis of 3 6.6% （3/82） ,and failure to diagno‐sis of 42 8.% （9/21） .Using echo attenuations ,correct diagnosis was 89 3.% ,misdiagnosis was 7 3.2% ,and failure to diagnose was 23 8.% .Using the resistance index ,correct diagnosis was 94 2.% ,misdiagnosis was 2 4.4% ,and failure to diagnose was 19 0.% .[Conclusion]Micro calcifications ,echo attenuation and RI≥0 7. suggest the breast calcification nodules in young women may have a higher correlationship with malignant breast diseases .
