摘  要:目的:通过对相关研究文献的系统评价和荟萃分析,探讨中国人醛糖还原酶(AR)基因C-106T多态性与糖尿病肾病(DKD)易感性的关系。方法:系统检索中英文科技期刊数据库,收集中国人AR基因C-106T多态性与DKD易感性的相关研究,按纳入与排除标准筛选文献、提取资料并评价纳入研究的方法学质量。采用Stata12.0软件进行荟萃统计分析,并检测敏感性及发表偏倚。结果 :共4篇研究、2271例样本纳入分析,其中DKD组711例、对照组1560例。荟萃分析结果显示,中国人AR基因C-106T多态性与DKD显著相关,T等位基因携带者发生DKD风险显著升高[OR=1.58,95%CI(1.13,2.22),P=0.008]。显性遗传模型:OR=1.814,95%CI(1.197,2.748),P=0.005;共显性遗传模型:OR=1.744,95%CI(1.185,2.567),P=0.005。但由于纳入研究间样本量差距较大,存在一定发表偏倚。而敏感性分析显示去掉任何一项研究对合并OR值影响并不显著。结论:AR基因C-106T多态性与中国人DKD的发生存在一定相关性;T等位基因携带者可能存在更高的DKD发生风险。objective:To evaluate the association of aldose reductase (AR)C-106T gene polymorphism with the risk of diabetic kidney disease (DKD)in Chinese with type 2 diabetes,we conducted this meta-analysi.Meth- ods:Literatures were identified from PubMed,EMBASE,Cochrane Library,CBM,CNKI,Wangfang and VIP database on July 1,2015,and eligible reports were evaluated.Statal2.0 software was used to do the meta-anal- ysis,and the detection of sensitivity and publication bias.Results:Four studies and 2271 individuals were in- cluded in the final analysis.Significant association were found between AR C-106T gene polymorphism and susceptibility of DKD in patients with type 2 diabetes.T allele significantly increased the risk of the occur- rence of DKD [OR=1.58,95%CI(1.13,2.22),P=0.008].As compared T allele with C allele,there were signifi- cant differences in the susceptibility both in dominant model [0R=1.814,95%CI (1.197,2.748),P=0.005]and codominance model [0R=1.744,95%CI (1.185,2.567),P=0.005].However, due to large sample size gap between the included studies,there was a publication bias.The sensitivity analysis showed that if any study removed, there was no influence on the pooled OR.Conclusion:The T allele of AR C-106T gene polymorphism may in- crease the susceptibility of DKD in Chinese with type 2 diabetes.
