摘 要:目的探讨兔甲状腺功能亢进性心肌病不同左心室构型的血浆B型脑钠肽(brain natriuretic peptide,BNP)浓度变化。方法新西兰纯种白兔30只,按随机数字表法随机分为对照组(n=10)和实验组(n=20)。对照组兔每日腹腔注射0.9%氯化钠溶液至实验结束。实验组兔腹腔注射左甲状腺激素(45μg·kg-1·d-1×28 d),建立甲状腺功能亢进动物模型,该组兔依据Ganau超声参数分为两型:离心性肥厚(eccentric hypertrophy,EH)亚组(n=11)与向心性肥厚(concentric hypertrophy,CH)亚组(n=7)。对各组兔均行超声心动图检查,并测量血浆BNP浓度。结果 EH组左心室质量指数(LVMI)明显增加(P<0.01),而相对室壁厚度(RMT)与对照组比较,差异无统计学意义(P>0.05);左心室射血分数(LVEF)、短轴缩短率(FS)比对照组明显减低,差异有统计学意义(均P<0.01)。CH组左心室质量指数及相对室壁厚度明显增加(均P<0.01);左心室射血分数、短轴缩短率与对照组比较,差异无统计学意义(均P>0.05)。EH组和CH组血浆BNP浓度明显高于对照组,差异有统计学意义(均P<0.01);EH组较CH组血浆BNP浓度明显升高,差异有统计学意义(P<0.01)。血浆BNP浓度与左心室质量指数呈正相关。病理学结果示EH组损伤较CH组重。结论兔甲状腺功能亢进性心肌病不同左心室构型的血浆BNP浓度具有不同程度的升高,并与病理损害程度基本一致。Objectives To study the concentration of brain natriuretic peptide(BNP) in different geometric patterns of left ventricle in rabbits with hyperthyroid cardiomyopathy. Methods Thirty New Zealand white purebred rabbits were randomly divided into two groups : control group(n =10) and experimental group(n =20). Sodium chloride injection was daily given in control group during the experiment. Hyperthyroid animal model was established by intraperitoneal injection of levothyroxine(L-Thy,45 μg·kg-1·d-1×28 d) in experimental group. According to the ultrasound parameters,the ventricular geometries in experimental group were further divided into two subgroups : eccentric hypertrophy(EH) subgroup(n =11) and concentric hypertrophy(CH) subgroup(n =7). Each group underwent conventional echocardiography. Plasma concentration of BNP in each group was evaluated. Results In EH subgroup,relative wall thickness(RWT) did not change significantly comopared with that in control group(P〈0.05), however RWT increased obviously in CH subgroup(P〈0.01). Plasma BNP concentrations in CH and EH subgroups were conspicuously higher than that in control group(all P〈0.01). Plasma concentration of BNP in EH subgroup was conspicuously higher than that in CH subgroup(P0.01). Plasma concentration of BNP was positively corelated with left ventricular mass index(LVMI). The pathological examination demonstrated that damages were more severe in EH subgroup than in CH subgroup. Conclusions Plasma concentration of BNP in the different geometric patterns of left ventricle in rabbits with hyperthyroid cardiomyopathy increases in varying degree and the elevation of BNP is in accordance with the degree of pathological injury.
